股骨干骨折不愈合的临床研究及治疗体会Clinical study and treatment experience of fracture nonunion of femoral shaft.pdfVIP

提 要 目的:本文通过收集股骨干骨折术后骨折不愈合病例，对患者骨折断端的骨愈合 状况及患肢的功能活动进行评价，进而对股骨干骨折术后不愈合治疗方法的临床效果 进行归纳与总结，为今后的临床实践提供更可靠更深层次的指导。 方法:本研究选择 20 例股骨干骨折术后骨折不愈合的病人，进行回顾性分析，治 疗方式采用更换内固定并自体髂骨髓内髓外植骨术。观察并随访患者骨折愈合状况以 及患肢功能，对结果进行统计学分析，评定治疗效果。 结果：对 20 例患者 18 例均一期骨性愈合，完全骨性愈合 5~8 个月，平均 6.5 个 月。 结论: 现代医学界对骨折认识的局限性与病人自身性因素（过早负重，骨折类型， 活动过多等）是股骨干骨折术后骨折不愈合的两个主要原因。更换内固定器械并以自 体髂骨植骨疗效确切。 关键词 股骨干骨折不愈合 自体髂骨植骨 四大原则 一期愈合 The Analysis and Treatment of the Nonunion of the Fracture of the Femoral Shaft Specialty：Orthopedics of TCM Author ：Xiao Lin Tutor：Associate Professor Li Gang Abstract Objective ：To evaluate the factors and the treatment of the postoperative nonunion of the fracture of femoral shaft. Method: In the clinical study, the 20 patients with postoperative nonunion of the fracture of femoral shaft receive the same treatment principles:In these cases,the internal fixation is replaced and autogenous bone graft is adopted . Results ：18 cases of 20 got osteal healing.The healing time lasts 5~8 months,average 6.5 months. Conclusion：Iatrogenic factors and patients own factors are the main factors of the nonunion of postoperative femoral shaft fracture.The replacement of internal fixation with the combination of the adoption of autogenous bone graft is effective in the treatment significantly.
