提 要 目的：探讨胫骨平台骨折三柱分型方法及其治疗分析。 方法：2009年3月至 2011年3月，62 例胫骨平台骨折应用三柱分型，其中有2 例单纯压缩骨折，37 例单柱骨折，20 例双柱骨折和 3 例三柱骨折。根据三柱分型选 择手术入路及内固定方式，外侧柱、内侧柱和后侧柱骨折分别使用前外侧入路、改良 正中入路和倒L形入路；对于双柱和三柱骨折则采取联合入路进行手术。术后随访并 评价临床效果，观察手术时间、平均骨折愈合时间、术后即刻与术后 12 个月的胫骨 平台内翻角及后倾角、膝关节活动度及 Lysholm 膝关节评分。 结果：单柱、双柱及三柱骨折的平均手术时间分别是 72.8 min、92.6 min、 106.1min，术后随访 12～32 个月，平均随访 21.6 个月，平均骨折愈合时间为 15.7 周，术后 12个月的Lysholm 膝关节评分平均为90.3分，测量术后即刻与术后12个 月X线片上的胫骨平台内翻角及后倾角，差异无统计学意义(P0.05)。 结论：基于 CT 的胫骨平台三柱分型方法对复杂胫骨平台骨折可以起到良好的手 术指导作用，切开复位内固定手术治疗复杂胫骨平台骨折可以取得良好的治疗效果。 关键词 胫骨平台骨折；骨折内固定术；分型；三柱 Analyzing the three column classification of tibial plateau fractures and their treatment Speciality：Orthopaedics of TCM Author ：Deng Wenjie Tutor：Professor Zhang Junzhong Abstract Objective: To explore the Three-Column Classification method of tibial plateau fractures and the analysis of their treatment.
