精品论文 参考文献 探讨动态心电图与常规心电图诊断无症状心肌缺血的观察 大庆市人民医院 163316 摘要：目的研究分析无症状心肌缺血在动态心电图及常规心电图上的不同。方法：此次研究的对象是择取2013年10月到2015年10月这两年间于我院的陈旧性心肌梗死患者共100例，将其临床资料进行回顾性分析，分别为其先后进行常规心电图检测与动态心电图检测。对比两种心电图的检测结果。结果：100例患者中动态心电图对无症状心肌缺血的检出率为61%；100例患者中常规心电图对无症状心肌缺血的检出率为17%。动态心电图检出率远远高于常规心电图，统计学有差异（Plt;0.01）。结论：对于确诊为冠心病的无症状心肌缺血患者，使用动态心电图进行24小时检测可以有效提高临床检出率。 关键词：动态心电图；常规心电图；冠心病；心肌缺血；无症状 [Abstract] Objective：To study the difference of dynamic electrocardiogram and routine electrocardiogram in the analysis of the silent myocardial ischemia. Methods：the object of this study is to October 2015 this two years in our hospital old myocardial infarction in patients with a total of 100 cases，the clinical data were retrospective analysis selects October 2013，respectively the has routine electrocardiogram and dynamic electrocardiogram detection. Comparison of two kinds of ECG detection results. Results：the detection rate of 100 patients with silent myocardial ischemia was 61%. The detection rate of 100 patients with silent myocardial ischemia was 17%. Dynamic ECG detection rate is much higher than conventional ECG，statistically significant difference（Plt;0.01）. Conclusion：for the patients with coronary heart disease in patients with asymptomatic myocardial ischemia，the use of dynamic ECG for 24 hours detection can effectively improve the clinical detection rate. [Key words] dynamic electrocardiogram；routine ECG；coronary heart disease；myocardial ischemia；no symptoms 无症状心肌缺血是在无胸痛或其它心绞痛等同症状的情况下存在心肌缺血的客观证据。心肌缺血的客观证据包括心电图一过性ST段变化、心肌灌注缺损和可逆性局部室壁运动异常等。在县级医院心电图一过性ST段变化见于常规体表心电图和动态心电图。故本文选择动态心电图和常规心电图两种检测方式，试探讨二者在无症状心肌缺血方面的诊断价值。 1 资料与方法 1.1 一般资料 择取2013年10月到2015年10月这两年间于笔者所在县级医院的陈旧性心肌梗死患者共100例，其基本资料如下：①性别：男性患者67例，女性患者33例；②年龄：最小者65岁，最大者85岁，平均（76.15plusmn;8.38）岁。分别为100例患者先后进行常规心电图检测与动态心电图检测，为防止研究结果受到干扰，试验已将具有如下状况的患者排除在外：①电解质紊乱，有低钾血症症状；②有明确胸痛或心绞痛症状；③左心室与右心室具有明显的肥大；④严重心律失常，如心房颤动或扑动，快速性和缓慢性心律失常，房室传导阻滞及室内传导阻滞等。 1.2 方法 首先为所有患者做常规心电图检查，统一使用日本光电9130P心电图机检测，医生对其进行人工分析，之
