精品论文 参考文献 经尿道前列腺电切术后尿道狭窄原因分析及治疗体会 倪国平 汉源县中医医院外科 625302 【摘 要】目的：探讨经尿道前列腺电切（TURP）和双极等离子电切术（PKRP）术后尿道狭窄的原因并对其治疗方法作简要探讨。方法：对我院2014年6月至2015年12月诊断为良性前列腺增生行经尿道前列腺电切术（TURP）和双极等离子电切术（PKRP）的各91名患者，进行随访。结果：对手术时间、留置尿管时间、术后住院时间、术后尿 WBC 等相关数据进行分析，发现 PKRP组明显优于TURP组（P＜0.05）。进一步将手术时间、留置尿管时间、术后住院时间、术后尿 WBC等相关数据与术后半年尿道狭窄发生率进行 Logistic 回归分析，发现术后留置尿管时间是造成 TURP术后尿道狭窄的主要因素，留置时间越长，发生尿道狭窄的概率越高，而PKRP组各临床指标均对尿道狭窄的发生无明显的影响。结论：PKRP在治疗在术后尿道狭窄发生率方面明显低于TURP，有良好的应用前景；且尿道狭窄是TURP术后常见的并发症，其发生原因有尿路感染、术中操作及术后护理护理不当引起，术后随访早期选择合适的方法进行治疗是康复的关键。 【关键词】经尿道前列腺电切术；尿道狭窄；治疗体会 【Abstract】objective：To investigate the causes of urethral stricture after transurethral resection of prostate（TURP）and bipolar plasma（PKRP）and discuss the treatment methods.Method：From June 2014 to December 2015，91 patients with benign prostatic hyperplasia were treated by transurethral resection of the prostate（TURP）and bipolar plasma resection（PKRP），and the patients were followed up.Result：The operation time，indwelling catheter time，postoperative hospital stay，postoperative urinary WBC and other related data were analyzed and found that the PKRP group was significantly better than the TURP group（P lt; 0.05）.Further post operative time，indwelling catheter time，hospitalization time，postoperative urine WBC and other relevant data and postoperative half urethral stenosis incidence were analyzed by logistic regression analysis，were found after indwelling catheter time is the main factor influencing the urethral stricture after transurethral resection of the prostate，the indwelling time longer，have higher probability of urethral stricture and PKRP group in the clinical indicators were of urethral stenosis occurred without obvious effect.Conclusion：PKRP in treatment in postoperative urethral stricture incidence was significantly lower than that of TURP，good application prospect，and urethral stricture is after transurethral resection of prostate（TURP）is a common complication and the cause of urinary tract infection，intraoperative operation and
