精品论文 参考文献 老年人下肢骨科手术腰麻所需布比卡因浓度和剂量的临床研究 山东省潍坊市第二人民医院 摘要：目的：研究观察小剂量低浓度布比卡因腰硬联合麻醉在老年人下肢骨科手术中的麻醉效果和对循环系统的影响。方法：我们随机选择了2013年2-8月间的62例老年下肢骨科手术的病人，随机分为A，B两组，全部采用腰硬联合麻醉。A组组病人选用0.375%等比重布比卡因，注射1.5-2ml，布比卡因用量约为5.5-7.5mg：B组病人选用0.5%等比重布比卡因，注射1.5-2ml，用量为7.5-10mg。分别观察了A，B两组的麻醉效果，术中平均动脉压的变化以及A，B两组在麻醉过程中血管活性药物的使用比例。结果：AB两组病人麻醉均成功，手术医师对麻醉的满意度为100%。A，B两组麻醉对循环系统的影响有统计学差异（P＜0.05），表明B组血压波动幅度较A组大。A组有4例使用血管活性药物，占比11%；B组11例使用，比例为35%，提示A组循环更平稳。结论：在老年人下肢骨科手术中，低浓度（0.375%）小剂量（5.5-7.5mg）布比卡因能够达到较确切的麻醉效果，麻醉药用量小，并发症较少，对循环系统干扰也比较小。 关键词：布比卡因；腰麻；老年人；下肢骨科手术 【Summary 】：Purpose：The research observes low density cloth of small amount of to compare card because of waist the hard consociation anaesthetize at old people legs orthopedics anaesthesia effect within surgical operation and to the influence of the circulatory system.Method：We random chose 2013 for 2-8 months，62 patients of old-age legs orthopedics surgical operations of，random is divided into A，the B is 2 sets，all adopt waist hard consociation anaesthesia.The A set set patient chooses specific weight clothes like 0.375%，etc to compare card because of，inject a 1.5-2 mls，cloth compare card because dosage is about 5.5-7.5 mgs：The B set patient chooses specific weight clothes like 0.5%，etc to compare card because of，inject a 1.5-2 mls，dosage is 7.5-10 mgs.Observed A respectively，the B was two sets of anaesthesia effect，the average artery in Shu presses of variety and A，B 2 sets are in the process of anaesthetizing in the use proportion of afferent and live medicine.Result：AB two sets of patients anaesthetize all and successfully，surgical operation doctors satisfaction to the anaesthesia is 100%.A，B two sets of the influence of anaesthesia upon the circulatory system include a statistics to learn difference（Plt;0.05）and express a the B set blood pressure motion range more the A set is big.The A set has 4 uses afferent and live medicine and has to compare 11%；the B set is 11 uses，the proportion is 35%，hint one A set circulation is more steady.Conclusio
