精品论文 参考文献 阑尾周围脓肿行腹腔镜与开腹手术的疗效对比 王爱华1 郭云霞2 马永香1 唐绪坤1 王海军1 姜元顺1 刘文利1 　　（1丘市中医院 山东潍坊 262100 ;2安丘市人民医院 262100） 　　【摘要】目的:对比分析腹腔镜阑尾切除术（LA）和开腹阑尾切除术（OA）治疗阑尾周围脓肿的临床疗效。方法：回顾分析2008年1月至2012年12月98例患者行阑尾切除术治疗阑尾周围脓肿的临床资料，其中腹腔镜阑尾切除术（LA组）56例，开腹阑尾切除术（OA组）42例。比较两种术式的术中出血量、手术时间、肠功能恢复时间、切口感染、残余脓肿发生率、住院时间、住院费用、术后疼痛程度、切口满意度。结果：比较腹腔镜阑尾切除术组和开腹阑尾切除术组以上各指标差异均有显著意义（P＜0.05）。结论：腹腔镜阑尾切除术治疗阑尾周围脓肿和开腹阑尾切除术相比，具有创伤小、恢复快、并发症少和平均住院时间短等优点，是治疗阑尾周围脓肿较理想的手术方式。 　　【关键词】腹腔镜阑尾切除术 开腹阑尾切除术 阑尾周围脓肿 　　【中图分类号】R565.8 【文献标识码】A 【文章编号】2095-1752（2014）21-0038-02 　　【Abstract】Objective:Comparative efficacy of appendiceal abscess underwent laparoscopic and open operation. Comparative analysis of laparoscopic appendectomy (LA) and open appendectomy (OA) the clinical curative effect of the treatment of appendiceal abscess. Methods: Back in 2008 January to 2012 December analysis of the clinical data of 98 patients underwent appendectomy in the treatment of periappendiceal abscess, wherein laparoscopic appendectomy (LA group) 56 cases, open appendectomy (OA group) 42 cases. Of the two groups were compared intraoperative bleeding volume, operation time, recovery time of intestinal function, infection of incision, the incidence of residual abscess, duration of hospitalization, cost of hospitalization, postoperative pain,.Results: Comparison of laparoscopic appendectomy group and laparotomy appendectomy group above indexessignificantly (P lt; 0.05).Conclusions: Laparoscopic appendectomy in the treatment of appendiceal abscess and open appendectomy compared, with small trauma, quick recovery, less complications and average shorter hospitalization time etc, is an ideal operation treatment of periappendiceal abscess. 　　【Key words】 Laparoscopic appendectomy open appendectomy appendix abscess 　　阑尾炎（阑尾周围脓肿）的治疗过去多采用传统的开腹手术，随着腔镜微创技术的发展，腹腔镜阑尾切除术的适应证逐步扩大。我院自2008年1月至2012年12月对98例阑尾周围脓肿病人实施手术，其中腹腔镜阑尾切除术（LA）56例，开腹阑尾切除术（OA）42例。分析如下。 　　1 资料与方法 　　1.1 临床资料 　　根据临床症状、体征、实验室检查诊断为阑尾
