闪燃事故分享 Flare Fire Accident Share 2011 1 13 January 13 2011 事故描述Accident Description 19：00左右某公司加氢芳烃联合车间操作人员发现罐区内一罐的回流总阀内漏 At about 19:00, Dec. 23, a company Hydrogenation Aromatics Complex operator found that total reflux valve of a tank at tank farm leaked internally. 21：00左右操作人员联系承包商现场进行处理后进行系统投用过程中发现另外一个阀门阀芯脱落。 At about 21:00, after the operators contacted the contractor and fixed the problem at the site, it was found that trim fell from another valve when the system was putting into operation. 23：00左右在拆脱落阀芯阀门的压盖过程中发生闪燃，3名现场作业的承包商逃出现场但被烧伤，其中一人98% 3度烧伤2天后死亡。另外两人中的一人在事发10天后死亡，一人重度烧伤。 At about 23:00, flare fire occurred during the removal of bonnet from the valve with trim fallen, 3 contractor workers flied out of the site, but was burnt, one worker had level 3 burn on 98% of the body area, and died after two days. One of the other two workers died 10 days after the accident, the other was seriously burnt. 可能的原因分析Analysis of possible reasons 空气中高浓度轻烃遇点火源发生闪燃?? Flash fire occurs when?light hydrocarbon of high concentration contacts ignition source? 可燃气体来源 Flammable gas: 1、在拆阀盖过程中轻烃泄漏到环境中。导致作业环境周围可燃气体积聚。 During the bonnet removal, C5 leaked into the environment, resulted in the accumulation of flammable gas at the surroundings of the operation areas. 可能的点火源 Ignition source: ： 1、使用非防爆工具导致拆阀盖时产生火花 The use of non explosion tool resulted in the sparks when the bonnet was being removed. 2、承包商衣服产生的静电 Static electricity generated from contractor’s clothes 3、或其它火源、热源 Or other fire and heat source 发生泄漏和产生点火源的可能原因：作业风险评估不足、风险控制措施不到位，等 Other possible reasons: 1、拆阀盖作业是新增的作业范围，风险评估不足或作业控制措施不到位。(如：隔离、倒空、置换等） Temporary night work, risk assessment was not adequate, work control measures were not complete and in place. 2、作业人员知识欠缺，忽视了高浓度可燃气体环境下作业可能产生火灾、爆炸的风险。 The workers lacked the knowledge, ignored the risk that fire and explosion might occur in the high concentration environment 3、作业人员衣服非防火、防静电。
