* Nutrition is closely tied to skin care as a preventative measure in wound care. Malnutrition is defined as a deficiency of essential nutrients, or the improper absorption and distribution of essential nutrients. Protein Energy Malnutrition (PEM) is most detrimental to wound prevention and healing. Some additional methods of getting fluids into these patients: Popsicles and other frozen treats. Cups that are easy to hold Tasty “treats” Minimize coffee and increase water based intake. Additional methods of getting protein: Protein shakes and “Instant Breakfasts” Balance of protein rich foods. Elder patients have a difficulty breaking down protein and may become dehydrated, personal assessments need to be done before beginning a protein rich diet. Often REsidents may have other problems eating. Perhaps they are having problems with dentures fitting correctly etc. these need to be examined if they are not in taking as much food as they should, or if there is a sudden change in their intake. * Closed Pressure Ulcers or Eschar: They are not included in the 4-stage classification system. They present differently and may often conceal a very serious, deep and sometimes potentially fatal lesion. * * * A Stage IV pressure ulcer cannot become a Stage III, Stage II, and/or subsequently Stage I. When a Stage IV ulcer has healed it should be classified as a healed Stage IV pressure ulcer not a Stage 0 pressure ulcer. It can take up to one year for the wound to restore to 80% of tensile strength. * * Support surfaces include bedding and foam, as well as clothes as protective shields, pillows etc. * Each specific wound and each phase of wound healing will have specific characteristics relating to the management of moisture. Advanced dressings available today have been created in response to the understanding of the importance of moisture. * It is recommended that the educator or facilitator review the specific documentation with the staff.
