The German doctor, concerned about these complications, decided he must experience the effects himself to truly understand, and hopefully prevent, them. He persuaded his colleague, Dr August Hildebrandt, to assist. The plan was for Hildebrandt to inject half a millilitre of cocaine into Biers spine. Hildebrandt would then assess Biers sensitivity to pain and determine how long it took for the anaesthetic effect to set in. The first attempt proved disastrous. After inserting the needle into Biers spine, Hildebrandt struggled to connect the syringe. As a result, a significant amount of Biers cerebrospinal fluid leaked out through the needle along with most of the cocaine. Undaunted, Hildebrandt immediately volunteered himself for the experiment. This time the injection was successful. Within minutes Hildebrandt reported a feeling of warmth in both legs. For the next 45 minutes, Bier tested Hildebrandts ability to feel a variety of sensations including pin-pricks, pinching, tickling and stabbing. The anaesthetic was working. Being thorough, Bier also applied a burning cigar to his colleagues thigh (it felt warm but not painful), crushed his skin with forceps (no pain there) and administered a few blows to the shin with an iron hammer (no pain, although Hildebrandt did begin to sweat).
