PCT和CRP在婴儿败血症早期诊断中的效果比较 1714Chinese Journal of Medicinal Guide 2011 Volume 13 No.10 Serial No.96 1 2 贺佩如 , 刘益成 1 2 邵阳市中心医院PICU , 儿科急诊 , 邵阳 【 摘 要 】 目 的: 探 讨PCT 和CRP 在 婴 儿 败 血 症 早 期 诊 断 中 的 效 果 与 价 值 。 方 法 对 我 院 新 生 儿 病 房 收 治 的104 例 婴 儿 按 照 败 血 症 临 床 诊 断 标 准 分 为 观 察 组54 例, 对 照 组50 例, 所 有 患 儿 分 别 于 第3d, 第7d 检 测 血 清 中 PCT 和CRP 水 平, 通 过 观 察 分 析 各 指 标 对 新 生 儿 败 血 症 的特异性、敏感性等进行诊断。结果 观察组患儿入院检查 PCT和CRP水平均较对照组高,但采用抗生素治疗后水平下降显著, PCT 诊 断 敏 感 性 较CRP 高, 分 别 为92.5% 和71.8%,PCT 诊 断 特 异 性 也 高 于CRP, 分 别 为96.2% 和88.5% 。 结 论 PCT 对 于 婴 儿 败 血 症 早 期 诊 断 具有较高的敏感性和特异性,可作为婴儿败血症早期诊断的首选指标,对于疑诊新生儿败血症患者应同时参考多个指标。 【关键词】降钙素原; C反应蛋白; 婴儿败血症 + 【中图分类号】 R722.13 1 【文献标识码】A 【文章编号】1009--1714-02 Effects Comparison of PCT and CRP in the Early Diagnosis of Neonatal Septicemia 1 2 He Pei-ru , Liu Yi-cheng 1 2 PICU and Department of Pediatric Emergency , Shaoyang Central Hospital, Shaoyang, Hunan ,China 【ABSTRACT 】Objective:To discuss the effect and value of PCT and CRP in the early diagnosis of neonatal septicemia.Methods:104 infants were divided into the observation group with 54 cases and control group with 50 cases according to clinical diagnosis standards of septicemia.Test PCT and CRP level in the serum on 3rd and 7th day.Take the diagnosis by observing and analyzing the specificity and sensitivity of all indicators to the neonatal septicemia.Results:The level of PCT and CRP for patients in the observation group was higher than ones in the control group,but it significantly drop after the treatment by antibiotics.The diagnosis sensitivity of PCT was higher than the one of CRP,92.5% and 71.8% respectively;while the diagnosis specificity of PCT was also higher than the one of CRP,96.2% and 88.5% respectively.Conclusion:PCT has a high sensitivity and specificity in the early diagnosis of neonatal septicemia, which can be treated as the preferential indicator.We have to refer to multi indicators for the complicated cases with neonatal septicemia. 【KEY WORDS 】Calcitonin;C-react
