颈源性头痛的临床表现 疼痛的位置： 通常单侧，也可双侧。 起源于颈部，放射至额颞部，也可至眶上。 头痛的性质： 非搏动性、nagging. 持续时间： 可以是发作性，持续时间不定，数小时至数天。 也可为慢性波动性。 颈源性头痛的疼痛分布区 不同刺激点的疼痛分布区 诱发因素： 与颈部有关，颈部活动，机械刺激等。 伴随症状： 如恶心、呕吐、畏光等可出现，但较轻。 治疗措施及反应： 局部神经阻滞有效。 颈源性头痛的临床表现 颈源性头痛的三种类型 枕型 枕颞上颌型 眶上型 颈源性头痛的体格检查 颈部活动： 颈部小关节触诊： 压痛点的评估： 枕大神经（枕颞部） 枕小神经（胸锁乳突肌附着处） 第三颈神经根（C2-C3关节） 斜方肌前后缘和肌腹 C2/3关节面触诊 颈部检查 颈源性头痛的体格检查 颈源性头痛的辅助检查 放射学改变与头痛的关系尚不明确。 颈椎影像学并不能确定诊断，只是为了排除相关疾病。 颈源性头痛的诊断 临床诊断 介入性诊断 手法诊断 枕大神经阻滞诊断 颈源性头痛的诊断 Sjaastad 等认为颈源性头痛的诊断必须具有以下三个特征： １由头颈运动或姿势激发的单侧头痛； ２按压颈部引起头痛； ３单侧头痛放射到颈部和同侧肩部或上肢. Cervicogenic Headache International Study Group diagnostic criteria 符合1-5是可能诊断 符合另外三条中任一条是即很可能诊断。 颈源性头痛的诊断 这些特点并非颈源性头痛所特有。 也不足以同其他类型的头痛相鉴别。 放射影像学可无异常，或者其异常改变与非颈源性头痛重叠。 一致性较高的、可靠的特点： 疼痛起源于颈部并向额颞区放射 疼痛放射至同侧肩臂 颈部运动诱发疼痛 一致性缺乏的特点： 颈部活动受限 按压痛 颈源性头痛的诊断 颈源性头痛的诊断 国际头痛协会目前使用“颈部紊乱导致头痛”的分类标准。把颈2神经阻滞有效当作颈源性头痛的一个特征。 11.
