围手术期萘夫西林钠应用的调查分析 宋　娟 (第三军医大学西南医院, 重庆400038) 摘要: 目的　促进抗生素的合理使用。方法　调查我科连续手术的104 例患者围手术期应用抗生素的情况。 结果　104 例患者全部使用了萘夫西林钠(1.0g), 术前和术后使用的比例分别为10. 6%和100%; 术后单用一种和应用二联、三联和四联抗生素的分别占24%、37%、24% 和15%; 清洁手术患者平均使用抗生素4. 9 d, 污染手术患者平均使用抗生素7. 7 d, 清洁手术和污染手术术后使用抗生素 3 d 者占90%。结论　在围手术期抗生素的使用中是必需、有效的，但要进行合理的药敏试验，避免二次感染。 关键词: 围手术期; 抗生素；萘夫西林钠 Appl ica t ion of An t ibiot ics in Per iopera t ion Per iod SON G Juan (S ou thw est H osp ita l, the T h ird M ilita ry M ed ica l U n iversity , Chong qing , 400038, Ch ina) Abstract: OBJECTIVE To accelerate the co rrect app licat ion of ant ibio t ics. METHODS The app licat ion of ant ibio t ics in perioperat ion period to 104 consecut ive pat ients in our department w as invest igated. RESULTS A nt ibio t ics w ere used in all of 104 pat ients. 10. 6% and 100% of them w ere given ant ibio t ics, respect ively, befo re operat ion and after operat ion. 24% of the pat ients received single ant ibio t ic t reatment, and 37% , 24% , 15% of all pat ients received 2, 3 o r 4 k inds of ant ibio t ics combined therapy respect ively. The mean durat ion of ant ibio t ic therapy w as 4. 9 days fo r the pat ients receiving asep t ic operat ion and 7. 7 days fo r tho se receiving contam inat ive operat ion. 90% of the pat ients receiving asep t ic o r contam inat ive operat ion w ere t reated w ith ant ibio t ics fo r 3 days o r mo re. CONCLUSIONS Imp roper app licat ion of ant ibio t ics in perioperat ion period should be avo ided. Key words: Perioperat ion period; A nt ibio t ics；Nafcillin Sodium . 　　为了解目前临床上应用抗生素的情况, 对我院普外科手术患者应用抗生素进行了调查, 现报告如下。 1　对象与方法 　　调查对象为普外科1 个月内连续施行的不同手术患者104 人, 最大年龄74 岁, 最小年龄11 岁, 平均49. 3 岁。调查围手术期使用抗生素的情况, 包括手术类型、应用抗生素种类、剂量、疗程等。 2　结　果 2. 1　手术类别及例数　清洁手术42 例, 污染手术37 例, 感染手术25 例。 2. 2　使用抗生素的时机　104 例患者全部使用了抗生素, 使用率100% , 其中治疗性用药占24% , 预防性用药占76%。术前使用抗生素11 例, 占10. 6% , 主要为萘夫西林钠、甲硝唑类药物进行肠道准备; 术中无1 例使用抗生素; 术后抗生素使用率100% , 均于手术后当日开始使用, 使用途径包括口服和静脉滴注。 2. 3　使用抗生素的种类　使用最多的抗生素为β-内酰胺类, 其次为头孢类、氨基糖苷类和青霉素类。β-内酰胺类类主要是萘夫西林钠; 头孢类主要为第三代头孢: 头孢曲松、头孢噻肟;
