103例胆道镜辅助下胆总管探查术后一期缝合的临床探讨 　　【摘要】 目的：探讨胆道镜辅助下胆道探查术后一期缝合胆总管的安全性、手术指征及手术方法。方法：选择性地对103例有胆道探查指征的患者，术中行胆道镜探查取石后做胆总管一期缝合，回顾性分析其疗效。结果：103例均获痊愈，5例发生胆瘘，3例复发胆道结石。随访3个月～3年，未出现胆道狭窄、胆道残余结石，无胆管炎、梗阻性黄疸、腹盆腔感染、急性胰腺炎等并发症。结论：只要严格掌握适应证、缝合技巧及技术要领，胆道镜下胆道探查术后一期缝合胆总管是安全的。 　　【关键词】 胆道镜； 胆道探查术； 一期缝合 　　Clinical Study of Primary Suture of 103 Cases with Common Bile Duct after Expedition Under Choledochoscope/LI Kun-lun， CHEN Xin， LI Bo， et al.//Medical Innovation of China，2015，12（10）：138-140 　　【Abstract】 Objective：To investigate the security， indications and methods of the primary suture of common bile duct after exploration under choledochoscope.Method：The therapeutic effectiveness of primary suture of common bile duct in 103 cases having the indications subject to exploration of common bile duct under choledochoscope was retrospectively analyzed.Result：All the 103 cases fully recovered.The bile leakage occurred in 5 cases and biliary calculi recurrence occurred in 3 cases.During the follow up of 3 month to 3 years，no residual calculus or stricture of bile duct was seen.No serious obstructive jaundice， cholangitis， pancreatitis or abdominal cavity infection was occurred.Conclusion：If the indication and the technology of bile duct suturation are prepefly mastered， the primary suture of common bile duct after exploration under choledochoscopy is safe. 　　【Key words】 Choledechoscope； Exploration of common bile duct； Primary suture 　　First-author’s address：Zhoukou City Central Hospital， Zhoukou 466000， China 　　doi：10.3969/j.issn.1674-4985.2015.10.045 　　胆总管切开探查+T管引流术是治疗胆总管结石或胆道蛔虫病的传统术式，经历了100多年的历史。但患者术后带管时间长，生活质量差，易出现逆行感染、T管脱落、结石复发等并发症，这些都是不容忽视的问题。胆总管探查术后一期缝合在临床上已有应用，但目前对该术式的效果评价仍有争议。笔者2010年3月-2013年12月行胆道镜辅助下胆总管探查取石术256例，选择性的对其中103行胆总管探查术后一期缝合，疗效可靠。 　　1 资料与方法 　　1.1 一般资料 本组103例，约占同期胆总管探查手术的40.2%，其中男女比例37∶66，年龄25岁～78岁，中位年龄55岁。术前均行彩色多普勒检查，少数患者行腹部CT或（和）MRCP检查，其中胆囊合并胆总管结石91例，5例合并左右肝管结石。18例为胆囊切除术后，85例为首次胆道手术。患者均有间断性右上腹疼痛病史，21例伴有轻中度黄疸。术前均有胆总管扩张（内径≥1.0 cm）。合并糖尿病者23例，高血压36例，全组无急性梗阻性胆管炎及急性胆原性胰腺炎病例
