乙型肝炎肝硬化患者发生慢加急性肝衰竭的影响因素分析 　　doi：10.3969/j.issn.1007-614x.2014.21.69 　　摘 要 目的：结合乙型肝炎肝硬化患者发生慢加急性肝衰竭的临床治疗数据，对其影响因素进行了深入研究。方法：2012年1月-2014年1月收治乙肝肝硬化慢加急性肝衰竭患者100例，分为观察组和对照组，分别对两组相同身体机能指标进行整理统计。结果：两组间总胆红素、血氨、血肌酐、血钠等身体机能指标对比差异具有统计学意义（P0.05），对照组死因大部分是并发症与器官衰竭。结论：血清总胆红素、血氨、血肌酐和血钠是乙肝肝硬化患者发生慢加急性肝衰竭的主要影响因素，并发症是导致患者死亡的主要因素。 　　关键词 乙型肝炎 肝衰竭 研究 主要因素 　　Analysis of influence factors of the incidence of acute-on-chronic liver failure in patients with hepatitis B liver cirrhosis 　　Zhao Mingzhi，Niu Weili 　　Department of Hepatitis，the Infectious Hospital of Hebi City，Henan 458000 　　Abstract Objective：Combined with clinical treatment data of the incidence of acute-on-chronic liver failure in patients with hepatitis B liver cirrhosis，to intensively study the influence factors.Methods：100 cases of acute-on-chronic liver failure in patients with hepatitis B liver cirrhosis were selected from January 2012 to January 2014.They were divided into the observation group and the control group.The same body function indexes of patients in two groups were respectively given statistics.Results：The difference of the total bilirubin，blood ammonia，serum creatinine，serum sodium of the two groups was statistically significant（P0.05）.The death causes of the control group were mostly complications and organ failure.Conclusion：Serum total bilirubin，blood ammonia，serum creatinine and serum sodium are the main influencing factors.The complication is the main factor to cause the death of patients. 　　Key words Hepatitis B；Hepatic failure；Study；Major factors 　　慢加急性肝衰竭是乙肝患者肝衰竭情况中的一种，死亡率较高，目前还缺乏特效型药物和治疗手段，但通过采取综合治疗措施，积极防治并发症，能有效降低死亡率。本文结合临床治疗数据，对乙肝患者慢加急性肝衰竭影响因素进行了深入探讨。 　　资料与方法 　　2012年1月-2014年1月收治乙肝肝硬化慢加急性肝衰竭患者100例，男89例，女11例；年龄11～80岁，平均（42.21±2.42）岁；住院时间7～120天，平均时间（45.62±2.24）天。以住院结束后患者的存活状况为标准，选取在住院过程中或住院结束后，肝功能有较大改善，相关乙肝肝硬化慢加急性肝衰竭症状体征基本消失的50例患者作为观察组，选取在住院过程中或住院结束后，因器官衰竭死亡的50例患者作为对照组。两组在年龄、性别等一般资料对比上差异无统计学意义（P0.05），具有可比性。 　　方法：针对两组临床治疗资料，分别整理治疗前后以及病情最为严重时3个阶段的身体机能各项数据及指标，对诊断为慢加急性肝衰竭时的各项数据进行分析，并且对两组临床治疗过
