多胎妊娠自然减胎对辅助生殖技术临床结局影响研究 　　【摘要】 目的：分析多胎妊娠自然减胎对辅助生殖技术（assisted reproductive technology，ART）临床妊娠结局的影响。方法：回顾性分析2010年1月-2014年12月于本中心行体外受精/卵胞浆内单精子注射（in vitro fertilization/intracytoplasmic sperm injection，IVF/ICSI）后双胎妊娠自然减胎共193例患者的随访资料，进行统计分析，评估自然减胎对助孕结局的影响。结果：1104例双胎妊娠中发生自然减胎183例，自然减胎率为16.58%（183/1104）；其中119例双胎自然减胎发生在妊娠前8周，占65.03%（119/183）。减胎后保留单胎的分娩孕周、新生儿体重与同期单胎妊娠比较，差异均无统计学意义（P0.05），但显著高于同期双胎妊娠，差异有统计学意义（P0.01）。结论：多胎妊娠自然减胎后可延长妊娠时间，增加保留胎儿的出生体重；妊娠8周后实施选择性多胎妊娠减胎术可减少手术干预率及并发症。 　　【关键词】 体外受精-胚胎移植； 多胎妊娠； 自然减胎； 选择性多胎妊娠减胎术； 妊娠结局 　　Study of Effects of Spontaneous Reduction of Multiple Pregnancies on Clinical Outcomes in Assisted Reproductive Technology/WU Chong-cong，CAI Gui-feng，YANG Gui-yan.//Medical Innovation of China，2016，13（18）：009-012 　　【Abstract】 Objective：To study the effects of spontaneous reduction of multiple pregnancies on clinical outcomes in assisted reproductive technology（ART）.Method：A total of 193 cases of spontaneous reduction in twin pregnancies conceived by in vitro fertilization/intracytoplasmic sperm injection（IVF/ICSI） in our center from Jan 2010 to Dec 2014 were prospectively analyzed.Result：183 cases of 1104 twin pregnancies occurred spontaneous reduction（16.58%，183/1104）.Among which 119 cases（65.03%，119/183） of spontaneous reduction in twin occurred before 9th week of gestation.The pregnancy duration and birth weight in remaining singleton fetus after spontaneous reduction in twin were similar to that of singleton pregnancy（P0.05），while the duration was significantly longer than that of twin gestation， and the birth weight was heavier（P0.01）.Conclusion：Spontaneous reduction of multiple pregnancies was associated with prolonged gestation and heavier birth weight for the remaining fetus.To perform selective multiple pregnant reduction after 9th week of gestation might reduce the probability and the followed complications. 　　【Key words】 In vitro fertilization and embryo transfer； Multiple pregnancy； Spontaneous reduction；Selective multiple pregnant red
