* In the RAO projection with no angulation, the main portion of the right coronary artery (RCA) appears to be well visualized. With 30 degree caudal angulation, the RCA is opened to view and two severe lesions are revealed. * Coronary artery bypass grafting (CABG) can be performed to overcome the obstructed or severely narrowed coronary artery. The aorta is linked to the blocked vessel distal to the site of obstruction by a blood vessel. The vessel used can either be the internal mammary artery or a vein, usually taken from the patient’s leg. Several grafts can be applied, and the anastomoses of the graft vessel can be used to link it to more than one coronary artery. This is called a “jump” or “skip” graft. If an internal mammary artery is used, it is sewn onto the diseased vessel distal to the lesion only, since it is already “attached” to the aorta anatomically. * PTCA - Percutaneous Transluminal Coronary Angioplasy A catheter is inserted through the artery, past the obstruction. A balloon on the catheter is inflated to compress the plaque and overstretch the vessel. PTCA causes a controlled injury to the vessel wall. In atherosclerotic lesions, plaque fracture and arterial wall stretching are the mechanisms enlarging the lumen. Enlarging the narrowed vessel restores blood and oxygen flow to the tissue. * Stents are mounted on a balloon catheter, which is advanced to the lesion site and then inflated. The balloon pushes the stent into the vessel wall.
