痒觉刺激对脑卒中后抑郁临床疗效观察 　　[摘要] 目的 观察痒觉刺激对卒中后抑郁的临床干预效果。 方法 将2015年10月～2017年4月新发脑梗死经评估确诊为卒中后抑郁的患者随机分为试验组和对照组，各30例，两组均采用内科基础治疗，试验组在康复训练的基础上进行痒觉刺激治疗，对照组进行单纯的康复训练。两组在入院24 h内、住院7 d、发病14 d、发病1个月和3个月时分别进行汉密尔顿抑郁量表（HAMD）及入院24 h内、住院7 d、发病14 d NIHSS测评。结果 试验组治疗14 d和1个月、3个月时HAMD评分分别为（12.93±3.63）分、（9.70±3.50）分、（5.93±4.88）分与对照组分别为（16.90±3.35）分、（11.67±3.47）分、（9.30±3.57）分进行比较，差异具有统计学意义（P0.05）。结论 痒觉刺激早期实施可有效干预卒中后抑郁，降低抑郁程度。 　　[关键词] 痒觉；卒中后抑郁；康复训练；神经递质 　　[中图分类号] R749.1 [文献标识码] B [文章编号] 1673-9701（2017）32-0093-04 　　[Abstract] Objective To observe the clinical intervention effect of itching stimulation on post-stroke depression. Methods The patients with new cerebral infarction who were diagnosed with post-stroke depression after assessment from October 2015 to April 2017 were randomly divided into experimental group （n=30） and control group （n=30）. The two groups were treated with basic medical treatment. The experimental group was treated with itching stimulation based on the rehabilitation training. The control group was underwent simple rehabilitation training. Hamilton Depression Scale（HAMD） within 24 hours of admission， on the 7th day of hospitalization， on the 14th day of onset， at 1 month and 3 months of onset and NIHSS evaluation within 24 hours of admission， on the 7th day of hospitalization， on the 14th day of onset were evaluated. Results There were significant differences in the HAMD scores between the experimental group（12.93±3.63），（9.70±3.50），（5.93±4.88）， respectively and the control group（16.90±3.35），（11.67±3.47），（9.30±3.57）， respectively on the 14th day， at 1 month and 3 months of treatment（P0.01， P0.05， P0.01）. There was no significant difference in NIHSS between the two groups on the 7th day of the hospitalization and the 14th day of onset. Conclusion Early implementation of itching stimulation can effectively intervene in post-stroke depression and reduce the degree of depression. 　　[Key words] Itching； Post-stroke depression； Rehabilitation training； Neurotransmitters
