糖代谢异常代谢综合征患者发生4型心肾综合征临床分析 　　【摘要】 目的 分析糖代谢异常的代谢综合征患者及在不同肾损害情况下4型心肾综合征的患病率。方法 选取2009年1月~2009年7月在中山大学孙逸仙纪念医院内科住院的合并T2DM或IFG、IGT的代谢综合征患者。统计肾损害的患病率及临床特征，依有无肾损害、肾损害的形式分析4型心肾综合征的患病率。结果 531例患者中52.73%发生肾损害，肾损害组冠心病、心功能不全患病率均较非肾损害组高（?P?0.05），蛋白尿组冠心病、心功能不全的患病率均显著高于非肾损害组（?P?0.05），肾功能下降组较非肾损害组冠心病患病率高（?P?0.01），心功能不全的患病率也较高（?P?0.05）。结论 糖代谢异常的代谢综合征患者肾损害的发生率高，糖代谢异常的代谢综合征患者合并肾损害后心血管事件患病率进一步增高。? 　　【关键词】 肾疾病； 代谢综合征； 空腹血糖受损； 糖耐量受损； 糖尿病； 心肾综合征?? 　　Clinical analysis of type 4 cardiorenal syndrome in metabolic syndrome patients with disorder of glycometabolism ?HOU Jing-tao,YANG Fang-fang,CAI Le-bin，LAI De-yuan?*,et al.?*The Second Affiliated Hospital of Sun Yat-sen University, Guangzhou 510120,China?? 　　【Abstract】 Objective To explore the prevalence of type 4 cardiorenal syndrome in patients with metabolic syndrome.Methods 531 patients in The Second Affiliated Hospital of Sun Yat-sen University with metabolic syndrome and disorder of glycolmetabolism were studied from January 2009 to July 2009.Investigated the prevalence of kidney damage and clinical characteristics of these patients,analysis the prevalence of type 4 cardiorenal syndrome in patients with metabolic syndrome.Results The prevalences of coronary atherosclerotic heart disease and chronic heart failure were higher in kidney damage group than in non-kidney damage group (?P?0.05),Compared with non-kidney damage group,the prevalence of coronary atherosclerotic heart disease and chronic heart failure were both higher in proteinuria group(?P?0.05),In reduced renal function group,the prevalences of coronary atherosclerotic heart disease and chronic heart failure were higher.Conclusions Kidney damage was common in metabolic syndrome patients with disorder of glycometabolism.If metabolic syndrome patients with dysglymetabolism had kidney damage,the prevalence of type 4 cardiorenal syndrome for them would increase.? 　　【Key words】 Kidney disease； Metabolic syndrome； Impaired fasting glucose； Impaired glucose tolerance； Diabetic mell
