脑钠素对急性心肌梗死疗效及预后影响 　　 　　 　　[摘要] 目的 观察脑钠素对急性心肌梗死的疗效及对预后的影响。 方法 将急性心肌梗死患者随机分为脑钠素组和对照组，分别给予静脉点滴脑钠素和单硝酸异山梨酯治疗，观察两组患者的心肌酶峰值，左室壁运动情况，左室射血分数及NT-proBNP水平变化，随诊3个月，观察两组患者的预后。 结果 脑钠素组的心肌酶峰值低于对照组［CK：(1234.76±812.34)μ/L vs (1678.67±958.36)μ/L；CK-MB：（118.49±74.6）μ/L vs （176.41±93.63）μ/L］（P0.05）；随访3个月后，NT-ProBNP水平明显低于对照组［（256±126）ｎg/L vs （432.1±138）ｎg/L］（P0.05）；室壁运动积分指数优于对照组（1.16±0.11 vs 1.31±0.16） (P0.05），左室射血分数高于对照组［(57±0.09) ％ vs （49±0.06）％］；3个月后，脑钠素组的主要心脏不良事件（MACE）的发生率明显低于对照组（P0.05）。 结论 脑钠素可以限制梗死面积???大，保护心功能及改善近期与远期预后。 　　[关键词] 脑钠素；急性心肌梗死；疗效观察；预后 　　[中图分类号] R542.2+2[文献标识码] A[文章编号] 1674-4721（2014）05（b）-0048-03 　　 　　The curative effect of brain natriuretic peptide for acute myocardial infarction and the influence on prognosis 　　ZHANG Yan-hua JIN Shu-hui LI Dong-ju 　　Department of Emergency,A Branch of Central Hospital of Jiao Coal Group in Henan Province,Jiaozuo 454100, China 　　[Abstract] Objective To observe the curative effect of brain natriuretic peptide for acute myocardial infarction and the influence on prognosis. Methods Patients with acute myocardial infarction were randomly divided into the brain natriuretic peptide group and the control group,and they were treated by brain natriuretic peptide and isosorbide mononitrate for intravenous drip respectively.The peak value of myocardial enzyme,left ventricular wall motion,left ventricular ejection fraction (LVEF),and change of NT-proBNP level were observed in both groups.After 3 months follow-up,prognosis in the two groups was observed.
