髓内及髓外内固定治疗股骨粗隆间骨折临床研究 　　[摘要] 目的 探讨髓内及髓外内固定治疗股骨粗隆间骨折的临床效果。 方法 选择2015年6月～2016年10月在我院内固定治疗的股骨粗隆间骨折患者80例的临床资料进行回顾性分析，其中髓内组40例患者采用PFNA治疗，髓外组40例患者采用DHS内固定治疗。比较两组的临床疗效、手术时间、术中出血量、切口长度、术后并发症情况等。 结果 髓内组手术时间较髓外组短，术中出血量更少，切口长度更短，差异有统计学意义（P0.05）；但髓内组优的比例高于髓外组（P0.05）。两组并发症比较差异有统计学意义（χ2=5.463，P0.05）。 结论 髓内及髓外内固定治疗股骨粗隆间骨疗效优良率无显著差异，但髓内内固定治疗创伤小，恢复更快，并发症更少。 　　[关键词] 髓内固定；髓外内固定；股骨粗隆间骨折；髋内翻畸形 　　[中图分类号] R683.41 [文献标识码] B [文章编号] 1673-9701（2017）09-0088-03 　　Clinical study on intramedullary and extramedullary internal fixation in the treatment of femoral intertrochanteric fracture 　　LI Yuxiang LIU Yong ZHANG Xuebing 　　Department of Orthopedics NO.1， Jilin Oilfield General Hospital， Songyuan 138000， China 　　[Abstract] Objective To investigate the clinical effect of intramedullary and extramedullary internal fixation in the treatment of femoral intertrochanteric fracture. Methods The clinical data of 80 patients with femoral intertrochanteric fractures who were treated with internal fixation in our hospital from June 2015 to October 2016 were retrospectively analyzed. Among them， 40 patients treated with PFNA were as intramedullary group and 40 patients treated with DHS internal fixation were as extramedullary group. The clinical efficacy， operation time， intraoperative blood loss， incision length， postoperative complications between the two groups were compared. Results The operation time of the intramedullary group was shorter than that of the extramedullary group. And the bleeding volume was less and the incision length was shorter in the intramedullary group， with statistically significant difference（P0.05）. However， the excellent rate of intramedullary group was higher than that of the extramedullary group（P0.05）. There was significant difference in complications between the two groups（χ2=5.463， P0.05）.
