近端防旋髓内钉与近端锁定钢板治疗老年股骨转子间不稳定骨折的比较观察 　　[摘要]目的 比较近端防旋髓内钉与近端锁定钢板治疗老年股骨转子间不稳定骨折的效果。方法 分析2012年3月～2017年4月我院收治的76例老年股骨转子间不稳定骨折患者的临床资料，依据固定方式不同分为近端防旋髓内钉治疗组和近端锁定钢板治疗组，每组各38例。比较两组患者的手术状况、临床疗效及术后并发症情况。结果 近端防旋髓内钉治疗组患者的手术切口长度、手术时间、术中出血量、术中引流量、住院天数、术后负重时间、骨折愈合时间均少于近端锁定钢板治疗组，近端防旋髓内钉治疗组患者临床治疗的优良率（94.3%）高于近端?i定钢板治疗组（78.9%），近端防旋髓内钉治疗组患者术后并发症的发生率（5.3%）低于近端锁定钢板治疗组（26.3%），差异有统计学意义（P0.05）。结论 近端防旋髓内钉治疗老年股骨转子间不稳定骨折患者创伤小，优良率高，术后并发症少，值得临床推广应用。 　　[关键词]近端防旋髓内钉；近端锁定钢板；老年；股骨转子间不稳定骨折；比较 　　[中图分类号] R683.42 [文献标识码] A [文章编号] 1674-4721（2018）2（a）-0100-03 　　[Abstract]Objective To compare the effect of proximal femoral nail anti-rotation and proximal locking plate in the treatment of gerontism unstable intertrochanteric fracture.Methods 76 cases clinical data of gerontism unstable intertrochanteric fracture patients treated in our hospital from March 2012 to April 2017 were analyzed，and divided into proximal femoral nail anti-rotation group and proximal locking plate group by different fixed mode，38 cases in each group.The operation condition，clinical curative effect，costoperative complications of the two groups were compared.Results The operative incision length，operation time，intraoperative blood loss，intraoperative drainage volume，hospital days，postoperative weight-bearing time，fracture union time of proximal femoral nail anti-rotation group were less than those of proximal locking plate group.The clinical treatment excellent rate （94.3%） of proximal femoral nail anti-rotation group was higher than that of proximal locking plate group （78.9%），the costoperative complications （5.3%） of proximal femoral nail anti-rotation group was lower than that of proximal locking plate group （26.3%），the differences were statistically significant （P0.05）.Conclusion In the treatment of the gerontism unstable intertrochanteric fracture patients by proximal femoral nail anti-rotation group，the injury is little，excellent rate is high，costoperative complications is less，and it is worth to be used. 　　[Key words]Proximal
