冠状动脉介入术后抑郁障碍及心理干预的研究 　　【摘要】 目的 研究冠状动脉介入术后冠心病患者的抑郁障碍情况及心理干预的效果。方法 采用Zung抑郁自评量表（SDS）对105例接受冠脉介入治疗的冠心病患者进行抑郁障碍调查评分。将105例患者随机分为干预组和对照组，在手术前后对干预组患者进行心理干预，对照组常规治疗。结果 术前两组患者抑郁发生率、SDS评分无显著差异。心理干预后干预组的抑郁障碍较对照组有明显改善，SDS评分分别为（36．54±4．25）和（46．17±6．42）（P0．01），手术前后对照组抑郁发生率无明显变化。结论 冠脉介入术后抑郁障碍发生率为35．6%，与术前比较无统计学差异，心理干预可以有效减轻冠脉介入治疗患者的抑郁障碍。? 　　【关键词】冠心病;介入治疗;抑郁;干预 　　 　　The study of depression after PCI and psychology intervene 　　 　　LI Fang-jiang,ZHANG Fan.Department of internal medicine of The first hospital Affiliated to hebei north university? 　　【Abstract】 Objective To discuss depression in coronary heart disease after PCI and the effect of psychology intervene.Methods To survey the depression of 105 cases coronary heart disease with PCI treatment and gread them according to SDS.Divided 105patients into intervene group and control group randomly.The patient in intervene group treat with psychology intervene before and after PCI.The patient in control group treat with normal treatment.Results There are no sagificent difference between two group in ratio of depression and gread of SDS.The depression in intervene group improve better than control group(the SDS grade is 36．54±4．25 and46．17±6．42 respectively P0．01）There are no sagificent difference before and after PCIin intervene.Conclusion The ratio of depression after PCI is 35．6percent Psychology intervene can reduce the ratio of depression in coronaryt heart disease with PCI.? 　　【Key words】Coronary heart disease;Intervent treatment;Depression;Intervene 　　 　　冠心病患者抑郁患病率为15%~20%，是普通人的2~4倍[1-2]，心脏介入手术是冠心病住院患者最常见的诊疗措施，而抑郁对冠心病患者的发展和预后有重要影响。目前有关冠状动脉介入诊疗手术与抑郁关系的研究报道较少，为了解冠心病患者心脏介入治疗前后的抑郁障碍和心理干预的效果，笔者对105例接受经皮冠状动脉介入术（PCI）患者进行了分析。 　　 　　1 资料与方法 　　 　　1．1 病例选择 选取2004年10月至2006年12月在河北北方学院附属第一医院心内科行PCI治疗的冠心病患者105例，按随机数字表法分为干预组和对照组。干预组57例，男38例，女19例，平均年龄(58．45±12．06)岁；对照组48例，男29例，女19例，平均年龄（60．34 ±9．25）岁。两组的性别、年龄职业及文化程度无明显差异。 　　1．2 抑郁障碍评定 抑郁障碍评定：采用Zung抑郁自评量表（SDS）评分，在患者手术前1~2 d和手术后7~10 d分别进行两次评定。由经过特殊培训的心内科医师完成，对于有阅读、理解困难（包括视力问题、文化层次低下等
