Imaging studies such as MRI give a structural view and physiological studies such as the sensory nerve conduction velocities test large myelinated fibers only. It is a well established fact that a significant number of people ASYMPTOMATIC for low back pain have abnormal lumbar MRIs. A normal or abnormal MRI in a patient with low back pain may or may not help you evaluate the source of the patients pain. Does a negative MRI or “normal” NCV greater than 40 meters per second indicate normal nerve function? No. A small fiber radiculitis may not have a disc protrusion on MRI and of course would be associated with a large fiber conduction loss. Sensory nerve conduction threshold electrodiagnostic tests provide an objective, functional as well as physiological evaluation of the entire sensory nerve. Large myelinated, small myelinated and unmyelinated fibers can be evaluated for painless sensory thresholds and pain tolerances. Sensory threshold measurements determine if the nerve is conducting. Sensory threshold measurements also neuroselectively evaluate the different fiber types that respond to vibration, temperature and pressure. The Neurometer?
