Fracture of the Humeral Shaft Definition ：Under surgical department neck 12cm to supracondyloid 2cm place bone fracture Good sends the spot ：1/3 next 1/3 on 1/3 Good sends the age: Young adults Attention ：The next 1/3 bone fracture easy to damage the nervus radialis, Next 1/3 bone fracture Yi Gu not company 三、移位(Displacement) Brachium backbone bone fracture end shifting besides with violence direction and body gravity related, concerns directly with muscles contraction. 四、临床表现与诊断 According to the flesh wound history, the symptom and the unique symptom, X-ray inspection may make the diagnosis 【治疗措施】  Therapeutic measure 保守治疗expectant treatment 手术治疗operation treatment 保守治疗 原则：功能复位则可 适应证: 移位不明显的简单骨折 有移位的中、下1/3骨折经手法整复可以达到功能复位标准的 固定6~8周，照片显示有初步骨痂后去除外固定，开始练习肢体活动。 手术治疗 适应症 Is unable to achieve or the maintenance function replacement Merges other spot damage Multistage or smashing bone fracture The bone fracture does not heal Merge brachium artery, nervus radialis damage 手术治疗 适应症 无法达到或维持功能复位的 合并其它部位损伤 多段或粉碎性骨折 骨折不愈合 合并肱动脉、桡神经损伤的 常见并发症Common complication 桡神经损伤The nervus radialis damages 血管损伤Blood vessel damage 迟延愈合或不愈合Delay heals or does not heal 谢谢!! * * 肱骨干骨折 Fracture of the Humeral Shaft 一、概述 (General Considerations) 定义：外科颈下1?2cm至髁上2cm处骨折 好发部位：中1/3 下1/3 上1/3 好发年龄：青壮年 注意：中下1/3骨折易损伤桡神经， 下1/3骨折易骨不连 二、病因与骨折类型(Etiology Types) 1. 直接暴力（ Direct violence ）： 上、中1/3骨折 粉碎、横形骨折 2. 间接暴力（ The indirect violence tumbles ）： 中、下1/3骨折 斜形、螺旋形骨折 3. 旋转暴力（ The revolving violence ）： 中、下1/3骨折 螺旋形骨折 肱骨干骨折 移位方向主要决定于骨折的部位 三、移位(Displacement) 1.三角肌止点以上 (上1/3骨折) 近端：向前、内 远端：向上、外 2.三角肌止点以下 (中1/3骨折) 近端：向前、外 远端：向上 3.下1/3骨折：与暴力、前臂和肘关节位置有关 肱骨干骨折 四、临床表现与诊断 (Clinical Findings and Diagnosis) 1. 外伤史 2. 一般症状：肿、痛、功能障碍 3. 特殊体征：上臂缩短、成角畸形、 异常活动、骨擦音 肱骨干骨折 肱骨干骨折X光片 肱骨干骨折并桡神经损伤、垂腕畸形 4. 注意合并桡神经损伤： 垂腕、垂指，旋后障碍，感觉障碍 5. X线检查 肱骨干骨折 四、临床表现与诊断 内固定 Internal fixation 加压钢板、 带锁髓内针 解剖：中下1/3后外侧有一桡神经沟 血管损伤 非正常愈合
