罗哌卡因联合舒芬太尼用于无痛分娩50例剖析 　　【摘要】 目的 探讨罗哌卡因联合舒芬太尼用于无痛分娩的效果、并发症及对产程和新生儿阿普加评分的影响。方法 回顾2007年50例罗哌卡因配合舒芬太尼椎管内麻醉行无痛分娩的病例资料，并随机选取同期50例未行无痛分娩的产妇作为对照组，对比2组产妇的剖宫产率、疼痛程度、产程、新生儿的阿普加评分，观察无痛分娩组的不良反应。结果 无痛分娩组剖宫产率低于对照组；疼痛方面，分娩过程中无痛分娩组疼痛明显轻于对照组，但缝合会阴切口时2组差别无显著性；对产程影响方面，活跃期无痛分娩组短于对照组，而第二产程无痛分娩组长于对照组；新生儿阿普加评分2组间差别无显著性；不良反应方面，宫缩乏力100%（50/50）,皮肤瘙痒80%（40/50），尿潴留30%（15/50），余未发现明显并发症。结论 罗哌卡因、舒芬太尼椎管内麻醉行无痛分娩效果确切，安全，但应注意宫缩乏力、皮肤瘙痒、尿潴留等不良反应及对产程的影响。? 　　【关键词】 罗哌卡因；舒芬太尼；无痛分娩? 　　? 　　The analysis of 50 cases of analgesic labor with ropivacaine and sufentanyl ZHOU Shui-sheng,Gu Jian,Chen Xiao-wei,et al. The 3rd Affiliated Hospital of Sun Yat-sen University,Guangzhou ,510630，China 　　 　　【Abstract】 Objective To explore the effect,complications and the influence on labor and the Apgar score of newborns of analgesic labor with ropivacaine and sufentanyl. Methods 50 cases of analgesic labor with ropivacaine and sufentanyl in 2007 were looked back to,50 parturients without analgesic labor in the same period was chosen as comparison group randomly. The cesarean section rate, degree of pain,labor and the apgar score of newborns between the two groups were compared,and the side effects of the analgesic labor group were observed. Results The cesarean section rate of the analgesic labor group was lower;the degree of pain during labor of the analgesic labor group was lower,but there was no significant difference between the two groups when stitching the incision.Compared to the comparison group,the active stage of the analgesic labor group was shorter,yet the second stage of labor was longer.There was no significant difference in the Apgar score of newborns between the two groups.The side effects of the analgesic labor group as following: uterine inertia100%（50/50）, itch80%（40/50），urinary retention30%（15/50）.Conclusion It is effective and safe to perform analgesic labor with ropivacaine and sufentanyl,but the side effects such as uterine inertia, itch and urinary retention and the influence on the labor should
