脉冲CO2点阵激光治疗痤疮瘢痕不良反应及并发症剖析 　　[摘要]目的：明确脉冲CO2点阵激光的不良反应及并发症。方法：回顾性分析48例接受脉冲CO2点阵激光治疗的患者的随访资料，评价治疗后不良反应、并发症类型及持续时间。结果：48例痤疮瘢痕患者治疗后红斑平均持续时间（6.1±2）天，痂平均持续时间（4.2±1.9）天，疼痛平均持续时间（2.3±1.2）天， Vas疼痛评分（3.3±2.1）分。46.2%的患者出现炎症后色素沉着，平均持续时间（32±7）天，术后并发症的发生率为22.9%，其中单纯疱疹发生率为6.2%， 炎症反应发生率为10.4%。结论：脉冲CO2点阵激光治疗后不良反应及并发症均为短期自限性。 　　[关键词]CO2点阵激光；不良反应；并发症；痤疮瘢痕 　　[中图分类号]R619+.6 [文献标识码]A [文章编号]1008-6455（2013）01-0084-02 　　Side effects and complications of ablative fractional CO2 laser in treating acne scar 　　LIU Yan-hua1，2，YANG Hui-lan2，CHEN Hu-lin2，LIAO Chun2，LI Xue-mei2 　　（1.Graduate School of Southern Medical University，Guangzhou 510515，Guangdong；2.Department of Dermatology，General Hospital of Guangzhou Military Command of PLA， Guangzhou， 510010，China） 　　Abstract： Objective We conduct the study of adverse effects and complication of ablative fractional carbon dioxide laser resurfacing. Methods Forty-eight treatments were studied. This retrospective study involved the analysis of records of patients treated with ablative fractional carbon dioxide laser. Type and duration of side effects and complications were analysed.Results The average duration of erythema was （6.1±2） days while that of scabs was （4.2±1.9）days.Average pain was （3.3±2.1）days，the Visual Analogue Scale （VAS） was（3.3±2.1）.46.2% patients had post inflammatory hyperpigmentation （PIH），the average duration was （32±7）days，2 of the treatment lasted 3 months.Complications were reported for 22.9% of the 48 treatments， as follows 6.2% facial herpes，10.4% inflammatory reactions.Conclusion All the sides effects and complications are resolved quickly. 　　Key words： ablative fractional carbon dioxide laser、Side effects、complication、acne scar 　　点阵激光以微细的点状光束扫描皮肤，在剥脱部分表皮的同时，引起真皮浅层胶原热变性而刺激新胶原再生，做为激光治疗的新模式在改善痤疮瘢痕和光老化方面，已成为临床治疗的有效手段[1-3]。我们采用回顾性分析方法就二氧化碳点阵激光治疗后的不良反应及并发症进行分析，以求更客观地为患者提供术前咨询及治疗方案。 　　1 资料和方法 　　1.1临床资料：48例均为2011年3月～2012年6月就诊于广州军区广州总医院皮肤科门诊的中至重度痤疮瘢痕患者。妊娠妇女、有日光暴晒史、有光敏药物服用史的患者排除。皮肤类型均为Fitzpatric Ⅳ型，年龄（25.3±3）岁，其中男26例，女22例。
