球囊扩张-椎体后凸成形术治疗骨质疏松性椎体压缩骨折并发骨水泥渗漏分析 张 峥，张建党，邹德威，马华松，谭 荣，彭 军，汪 东，刘 涛（100101 北京，中国人民解放军第306医院骨科） [摘要] 目的 分析球囊扩张-椎体后凸成形术（PKP）治疗骨质疏松性椎体压缩骨折骨水泥渗漏情况。方法 收集本科2007年12月至2010年12月采用PKP治疗椎体压缩性骨折患者138例，男性41例，女性97例，平均75．3（56～87）岁。其中单椎体骨折115例，双椎体骨折19例，三椎体骨折2例，四椎体骨折2例；共167个椎体，其中T6 3个，T7 4个, T8 4个，T9 6个, T10 6个，T11 21个, T12 43个, L1 45个, L2 23个,L3 7个，L4 5个。结果 共12例(8.7％)患者发生骨水泥渗漏，其中渗漏至椎间隙5例，椎体前方4例，椎旁软组织1例，沿椎弓根穿刺通道渗入根管内1例，椎管内1例；未出现脊髓或神经损伤的症状和体征。结论 PKP治疗椎体压缩性骨折有一定的骨水泥渗漏率，存在造成严重后果的并发症的可能性，应重视骨水泥渗漏的积极预防。 [关键词] 经皮椎体后凸成形术；并发症；椎体压缩性骨折；骨质疏松症 Cement leakage following percutaneous kyphoplasty for osteoporotic vertebral compression fractures Zhang Zheng,Zhang Jiandang,Zou Dewei,Ma Huasong,Tan Rong,Peng Jun,Wang Dong,Liu Tao(The Department of Orthopaedics,The 306th Hospital of PLA,Beijing,100101,China) [Abstract] Objective：To investigate t Cement leakage following percutaneous kyphoplasty for osteoporotic vertebral compression fractures．Method：From December 2007 to December 2010．138 patients with vertebral compression fracture underwent PKP，there were 41 males and 97females，witll the age ringe from 56 to 87 years(average，75．3 years)；115 cases had one level fracture，19 cases had two levels fracture，2 cases had three levels, 2 cases had four levels involved，there were a total of 167 vertebrae involved，which including 3 in T6 ，4 in T7, 4 in T8，6 in T9, 6 in T10 ，21 in T11 , 43 in T12, 45 in L1, 23 in L2 , 7 in L3，5 in L4．The Cement leakage were reviewed retrospecfively．Result：Cement leakage 0CCoured in 12 CaSeS(8．7％): intervertebral space leakage in 5 cases，vertebral body leaking anteriorly in 4 cases,paravertebral leakage in 1 case，foramen leakage in 1 case,spinal canal extravasation in 1 case；No serious complications were noted．Conclusion：cement extravasation may occur during PKP, which may cause serious results．More attention should be paid On the prevention of cement extravasation． [Key words] Percutaneous kyphoplasty；Complication；Vertebral compression fracture; Osteoporosis Corresponding a
