* * This triangle represents our failures in safety. View this as an iceberg floating in the sea. The horizontal dashed line indicates the “water line”; those items above the line are visible, those below are not. Fatalities and serious injuries are certainly visible. Some minor injuries are visible, but many are not, in that they, as well as near misses, are often not reported. Traditionally, most organizations do not have visibility of at-risk behaviors. As you investigate the items further down the triangle, you move from being proactive instead of reactive. That is, you identify potential causes of injuries before they occur, rather than waiting until after they’ve resulted in an injury. If we can’t identify minor injuries, near misses, and at-risk behaviors, then we can’t work to prevent such incidents from happening again. In addition, the items further down the triangle are more prevalent. That is, near misses and at-risk behaviors occur more frequently than injuries. Therefore, investigating these lends more opportunities to discover and change things that impact safety.
