门脉高压性胃病病理初步分析研究生：贺启贵 门脉高压性胃病病理初步分析 研究生：贺启贵 导师：王全红教授主任医师 [摘要]目的：探讨门脉高压性胃病(Portal hypertension gastropathy PHG)的病理特点，分析对PHG的影响因素。材料 方法：对85例乙型肝炎所致肝硬化PHG患者(PHG组)的胃粘 膜活检组织进行幽门螺杆菌(Helicobacter pylon I IP)特殊染色， 乙型肝炎病毒(Hepatitis virus B HBV)表面抗原(HBsAg)和核 心抗原(HBcAg)免疫组织化学染色，光、电镜下观察；同时对 PHG组患者肝组织进行HBsAg和HBcAg免疫组织化学染色。正常 对照组40例胃粘膜活检组织光、电镜下观察，幽门螺杆菌特殊 染色。结果：PHG组胃粘膜血管充血扩张、内皮细胞连接松散、 粘膜上皮和间质水肿是PHG共同的病理改变。PHG组幽门螺杆菌 感染率为77．8％(66／85)，部分病例壁细胞增生(24／85)，胃粘 膜乙型肝炎病毒HBsAg和HBcAg免疫组织化学染色均为阴性，肝 组织免疫组织化学染色，HBsAg阳性80(94％)例，HBcAg阳性 83例(98％)：对照组幽门螺杆菌感染率为30％，PHG组显著高于 l 出受曼盎蠢墨 出受曼盎蠢墨 21蛆墨塑主茎焦蓬塞 对照组(pO．005)。结论：PHG有其较特征性的病理改变，幽门 螺杆菌感染和高胃酸可能是影响PHG的重要因素。HBV感染对PHG 影响的证据不足，有待于进一步研究。 [关键词]：高血压，门静脉； 胃病； 幽门螺杆菌； 乙型肝炎病毒 Pathological Pathological analysis of Portal hypertension gastropathy Tutor：Prof．Wang Quan Hong IABSTRACTl Objective：To explore the pathological characters of Portal hypertension gastropathy(PHG)，and to analysis the affecting factors of PHG．Methods：Gastric mucosal tissues from 85 patients with PHG(PHG group)were observed under light and electron microscopy,and immunohistochemistry and special stain were also performed．In the meantime，all hepatic tissues from this group were performed immunohistoehemistry stain．‘In contrast with PHG group， Gastric mucosal tissues from 40 normal cases(control group)were observed under light and electron microscopy,special stain were also performed．Results：Vascular changes of the gastric mucosa was the basic pathological characters of PHG，and helicobacter pylori(HP)infection rate WaS 77．8％in PHG group．oxyntic cells were hyperplasia in some cases，immunohistochemistry stain of 出重堡壁鑫茎 出重堡壁鑫茎 29Q≥墨瑟主芏焦熊=墨 HBsAg and HBcAg Was negative in the gastric mucosa,the positive rate of HBsAg and HbcAg immunohistochemistry stain in the hepatic tissues Was 94％and 98％，respectively．The HP infection rate in control group was 30％，the HP infection rate in PHG group Was obviously higher than those in control
