预防人流术后宫腔粘连几种措施的对比研究 【摘要】 目的 探讨人工流产术后口服药物以及宫腔注射玻璃酸钠预防宫腔粘连的临床疗效。方法 随访观察施行人工流产终止早期妊娠的患者572例，流产次数为两次及两次以上的患者。按照随机及自愿的方法分为人工周期组、优思明组（屈螺酮炔雌醇片）、玻璃酸钠组及对照组。结果 人工周期组子宫内膜厚度与其余三组比较差异有统计学意义（P﹤0.05），其余三组比较差异无统计学意义（P0.05）；人工周期组与优思明组月经复潮时间比较，差异无统计学意义，两组与玻璃酸钠组及对照组比较差异均有统计学意义；玻璃酸钠组与对照组月经复潮时间比较差异无统计学意义；三种预防方式均能明显降低术后宫腔粘连的发生（P﹤0.05）。结论 人工流产后即刻给予人工周期或者优思明能够有效促进子宫内膜的修复，减少宫腔粘连的发生。但对于依从性较差的患者，仍首选优思明作为人流后预防宫腔粘连发生的首选药物。 【关键词】 人工流产；宫腔粘连；人工周期；优思明；玻璃酸钠 中图分类号：R715.2 A Comparative Study Of Several Measures To Prevent The Adhesion Of Intrauterine Adhesions After Artificial Abortion 【Abstract】Objective To investigate the clinical efficacy of oral administration and sodium hyaluronate in the prevention of intrauterine adhesions after artificial abortion.Method Follow up observation of 572 cases of patients with early pregnancy termination of artificial abortion, the number of abortions for the two time and more than two patients. According to the method of random and voluntary divided into artificial cycle,Yasmin group(flexion drospirenone ethinyl estradiol tablets), sodium hyaluronate group and control group.Result The difference between artificial cycle endometrial thickness and the rest of the three groups had significant difference(P﹤0.05),the difference compared to the other three groups no statistical significance(P0.05); artificial cycle and Yasmin group menstrual recovery time,the difference was not statistically significant,the two groups with sodium hyaluronate group and control group comparison showed statistically significant differences;sodium hyaluronate group and control group menstrual recovery time difference no significant;three prevention methods can significantly reduce the occurrence of postoperative uterine cavity adhesion(P﹤0.05).Conclusion After artificial abortion immediately give artificial cycle or Yasmin can effectively promote endometrial repair and reduce the incidence of intrauterine adhesions.
