PAGE PAGE 1 2017-03-29董 手法为主综合治疗椎动脉型颈椎病疗效观察 吴其友，李 伟，王 蛟，张 智，舒文韬，赵立勇，罗建平 （四川省中医药科学院中医研究所 骨科，四川 成都 610031） 【摘要】目的：观察手法为主综合治疗椎动脉型颈椎病的临床疗效。方法：自2014年7月至2015年9月共收集60例椎动脉型颈椎病患者，随机分为观察组与对照组，每组各30例。观察组采用手法配合电针、口服颈痛颗粒治疗；对照组采用电针配合口服颈痛颗粒治疗。治疗后第7天、第14天、第21天分别观测2组患者颈肩痛、眩晕、猝倒等主观症状和Jackson’s试验、Spling’s 试验、椎动脉扭曲试验等客观体征及经颅多普勒超声（TCD）血流速度变化等指标并比较临床疗效。结果：治疗后2组患者主观症状及客观体征等指标的改善情况有显著性差异（P＜0.05）；观察组椎-基底动脉平均血流速度较对照组改善明显（P＜0.01）。结论：手法为主综合治疗方案疗效确切，能明显改善患者的临床症状，值得临床推广应用。 【关键词】颈椎病；椎动脉型；手法治疗；电针；颈痛颗粒 【中图分类号】R681.5+5 【文献标识码】A DOI:10.11851/j.issn.1673-1557.2017.03.0 Therapeutic effect of comprehensive treatment dominated by manuduction on cervical spondylosis of vertebral artery type WU Qiyou, LI Wei, WANG Jiao, ZHANG Zhi, SHU Wentao, ZHAO Liyong, LUO Jianping （Department of Orthopedics, Chinese Medicine Research Institute of Sichuan Provincial TCM Academy, Chengdu 610031,China） 【Abstract】Objective: To investigate the effects of comprehensive treatment dominated by manuduction on cervical spondylosis of vertebral artery type(CSA). Methods: 60 patients suffering from CSA were randomly divided into two groups from July 2014 to September 2015, 30 cases in each group. Patients in observation group took Jingtong granule and received manuduction combined with electro-acupuncture while those in the control group took Jingtong granule and received electro-acupuncture. 7 d,14 d and 21 d after treatment, subjective symptoms (neck and shoulder pain, dizziness, cataplexy),objective signs(Jackson’s test, Spling’s test, vertebral artery distortion’s test) and blood flow rate detected by transcranial Doppler sonography were compared between the two groups. Results: Statistical analysis indicated that there were significant differences in the improvement of subjective symptoms and objective signs between the two groups (P＜0.05);the observation group showed better average blood flow rate of vertebral and basilar artery than the control group（P＜0.01）.
