国家卫计委临床药师培训基地:千佛山医院内分泌专业带教老师：李妍副主任药师 第十期学员：刘杨从 2015.05.19 邮箱163.com 二甲双胍作用机制文献阅读报告 主要内容 一、问题的提出 二、文献查找及阅读评价 三、结论 二甲双胍应用发展史 1927年，马勒（Muller）等，提取自草本植物的山羊豆碱可显著降低糖尿病患者的血糖，但对正常人血糖几乎无影响。 1957年，斯特恩（Sterne）等，经山羊豆碱经化学改造得到的二甲双胍对糖尿病的血糖控制有效，并具有良好的耐受性。（Glucophage，即格华止） 1959年，Sterne等对接受胰岛素治疗的2型糖尿病（T2DM）患者使用二甲双胍，发现为控制血糖水平所需的胰岛素用量大大减少，从而开启了T2DM联合治疗的新纪元。 1961年，兰伯特（Rambert）等，发现血糖控制不佳的T2DM患者接受二甲双胍治疗后，血糖可显著降低，而血糖控制良好者和健康人服用二甲双胍后血糖变化幅度很小这表明:二甲双胍控制血糖的机制是对抗高血糖而不导致低血糖，同时也验证了二甲双胍具有较好的安全性。 1995年，二甲双胍被美国FDA正式批准用于治疗T2DM。 1998年，具有里程碑意义的UKPDS（英国前瞻性糖尿病研究）证实，二甲双胍具有明确的心血管保护作用，在循证医学上将二甲双胍推向了新的高度。基于UKPDS等循证医学证据，2005-2015年，国际糖尿病联盟（IDF）、欧洲糖尿病研究学会（EASD）、美国糖尿病学会（ADA）等主要国际指南均推荐二甲双胍作为T2DM的一线和基础治疗药物。 二甲双胍药理作用研究进展概况 1.Metformin has several mechanism of action： it inhibits the hepatic glucose output and in turn decreases the hyperinsulinemia. 2.It provides survival benefits, and is potentially helpful in aging and aging-related disorders as proved by rodent studies, and also has potential in treatment of heart failure as shown by case–control and experimental animal studies. 3.Metformin also ameliorates inflammation and can be used in Inflammation based disorders. 4.Metformin can be used to prevent obesity and metabolic syndrome and is currently being used as front line therapy in polycystic ovarian syndrome; the leading cause of infertility among reproductive age females. 5.Its role in NAFLD is also investigated and several human based studies have proven its efficacy over other treatments. 6.Metformin has potential advantage as a chemotherapeutic for cancer as previous studies have established relation between metformin usage and decrease risk of various cancers. 一、问题的提出 二、文献查找及阅读评价 检索字段: a.主题 =二甲双胍and β细胞and作用机制 b.篇名=二甲双胍and AMPK c.篇名=二甲双胍and SIRT 检索数据库及策略： 1.CNKI中:路径a 阅读文献 筛选出价值文献 2.pubmed或CNKI中 回溯参考文献 二、文献查找结果 [1]刘率男,刘泉,孙素娟,侯少聪,王悦,申竹芳.二甲双胍对2型糖尿病KKAy小鼠胰岛β细胞功能的影响及初步作用机制探讨[J].药学学报,2014,
