Methods: Review and analyze clinical materials of 18 pathologically confirmed patients suffering from breast cystosarcoma phyllodes. Results: Breast cystosarcoma phyllodes occurs mainly in the population inbetween 10-year-old and 40-year-old and it has been characterized as the onset of painless breast mass. Not even one definite diagnosis could be made for all 18 patients until after surgical operation. Among the 18 patients, 13 underwent extended resection, another 3 underwent modified mastectomy, and the remaining 2 accepted simple mastectomy. The overall prognosis is good with local recurrence rate at 11.1% (2/18). Conclusion: Auxiliary examination is not of much help towards the conclusive diagnosis of breast cystosarcoma phyllodes, which has to rely on pathological examination. Extended resection is the major method for the treatment of breast cystosarcoma phyllodes and secondary operation can be still effective against local recurrence. 　　Key words Breast cystosarcoma phyllodes; Diagnosis; Treatment 　　乳腺叶状囊肉瘤是一种双相分化的肿瘤，由良性的上皮成分和间叶成分组成，是一种少见的纤维上皮性肿瘤，占乳腺肿瘤的0.3%～0.9%，占乳腺纤维上皮性肿瘤的2.5%[1]。1995-2006年我科共手术治疗乳腺肿瘤1 254例，经病理证实乳腺叶状囊肉瘤18例，占1.4%（18/1 254）。现就诊断和治疗探讨如下。 　　1 资料与方法 　　1.1 临床资料 本组18例，全为女性，年龄12～44岁，平均21.6岁，其中未婚女性占61.1%（11/18），已婚女性占38.9%（7/18）。病程3月～10年，平均23月。均以无痛性乳房肿块就诊，13例（72.2%）肿块短期内迅速增大，其中61.5%（8/13）分别与月经初潮、妊娠、哺乳有关。肿块全为单发，位于外上象限9例（50.0%），内上象限3例（16.7%），外下象限2例（11.1%），乳腺中央2例（11.1%），全乳2例（11.1%）。肿块最大直径3～15cm，平均6.5cm。肿块均未发现与皮肤和胸肌粘连，未发生破溃，出现局部皮肤血管怒张3例（16.7%），同侧腋淋巴结肿大2例（11.1%）。 　　术前细针穿刺细胞学检查10例次，均未发
