The learning contract forms the basis for the mid-term as well as the final evaluation. 　Procedure: Responsibility for completing the contract is shared between the student and the field instructor. Listed activities are required. In addition, the student must add learning activities specific to the setting under each objective. The contract is negotiated, completed, and signed by the student and field instructor and approved by the field liaison. It is reviewed /revised at midterm and may be further revised as needed.
