Methods Fifteen patients (6 women， 9 men) with small solitary pulmonary nodules(SPN) who underwent the examination with 64 - detector spiral CT (Lightspeed VCT， GE) were included， the diameter of nodules were less than 2cm. CT images were analyzed by same two radiologists respectively. The volumes of pulmonary nodules were measured and the morphologies were described separately with ALA software. Then， two-dimeional maximum diameter measurements and morphologies were also acquired in the GE workstation 15 days later. Bivariate method and the Spearman correlation coefficients were used to test the variability between three dimeional(3D) measurements and two dimeional(2D) ones; Paired-Samples T Test were used to test the accurate rate of diagnosis that was analyzed by 3D and 2D. Results Fifteen small SPN were histologically or clinically proved: peripheral pulmonary cancer 6 cases， metastatic tumo 5 cases， tuberculomas 2 cases， inflammatory pseudotumor 1 case， pulmonary vascular section mistake for SPN 1 cases. Three dimeional measurements between two reade were highly correlated with r of ， and the r was in the two dimeional measurements. The difference between the two correlation coefficients was significant. The three dimeional measurements between two reade had completely agreed with each other for small SPN. The accuracy rate of diagnosis analyzed by 3D was higher than by 2D， =，P =，Kappa=，coincidence rate was intermediate to high grade(～)，P =， he difference was significant between the accurate rate of diagnosis that was analyzed by 3D and 2D.
