Result: The hemorrhage， residence time of urinary canal， bladder washout time and length of stay in the treatment group were fewer than those in the control group， but there was no difference in operation there were difference in bleeding after operation， cystospasm， uroclepsia， dysuria and sexual disturbance in two groups except the rate of infection. Conclusion: The treatment of improved prostatectomy with remaining the urethra by the back of pubis is thorough ，the effect is ideal， complication after operation is fewer and the applicability is wide， so it is worth using in base hospital。 Key words： Hyperplasia of prostate; Madigan operation; Improved 我院自2002年1月至2006年12月用改良耻骨后保留尿道前列腺切除术(改良Madigan术)治疗良性前列腺增生(BPH)患者39例，与国内应用最广泛的耻骨上经膀胱前列腺切除术(SPCC术)比较，在治疗效果及术后并发症方面都有较好的表现，现报告如下： 1 资料与方法 临床资料:改良Madigan术组(治疗组)39例，年龄平均±岁，前列腺最大直径平均±，伴中叶增生突入膀胱者10例，国际前列腺症状评分(IPSS)平均±分，最大尿流率(Qmax)平均±，残余尿量287±58ml，术前有性功能者25例。SPCC术组(对照组)35例，年龄平均±岁，前列腺最大直径平均±，伴中叶增生者9例，I
