经S1骶髂关节螺钉固定时骶骨非安全区域的解剖学研究的开题报告 【摘要】 骶髂关节是人体骨盆的重要组成部分，经常需要进行手术干预。其中，经S1骶髂关节螺钉固定是一种常见的治疗方法。然而，该方法可能存在风险，因为在固定过程中如果螺钉穿过了骶骨的安全区域，可能会导致骶神经损伤等严重后果。因此，本研究旨在探索S1骶髂关节螺钉固定时骶骨非安全区域的解剖学情况，为该手术提供更精准的指导。 本研究计划招募30名符合条件的成年人进行骶骨CT扫描，确定骶骨的安全区域和非安全区域。同时，将根据具体手术情形，制定适当的螺钉进钻方案，并进行模拟手术实验，观察在不同方案下进钻时螺钉对骶神经的影响情况，以及不同进钻位置对骶骨强度的影响情况。最后，将对实验结果进行统计分析，得出S1骶髂关节螺钉固定时骶骨非安全区域的解剖学情况，并提出相应的临床指导建议。 本研究的结果可以为手术医师提供更全面、更科学的进钻方案，尽可能地避免手术中的并发症发生，提高手术成功率，保障患者安全。 【关键词】骶髂关节，螺钉固定，安全区域，骶神经损伤 【Abstract】 The sacroiliac joint is an important component of the human pelvis and often requires surgical intervention. Among them, S1 sacroiliac joint screw fixation is a common treatment method. However, this method may have risks because if the screw passes through the safe zone of the sacrum during fixation, it may cause serious consequences such as sacral nerve injury. Therefore, this study aims to explore the anatomical situation of the non-safe zone of the sacrum during S1 sacroiliac joint screw fixation, providing more precise guidance for the surgery. This study plans to recruit 30 eligible adults for sacral CT scans to determine the safe and non-safe zones of the sacrum. At the same time, appropriate screw drilling schemes will be developed based on specific surgery conditions, and simulation experiments will be conducted to observe the impact of screws on sacral nerves during drilling under different schemes and the impact of different drilling positions on the strength of the sacrum. Finally, the experimental results will be statistically analyzed to obtain the anatomical situation of the non-safe zone of the sacrum during S1 sacroiliac joint screw fixation and propose corresponding clinical guidance recommendations. The results of this study can provide surgeons with a more comprehensive and scientific drilling scheme to minimize the occurrence of complications during surgery, improve the success rate of surgery, and ensure patient safety. 【Key Words】sacroiliac joint, screw fixation, safe zone, sacral nerve i
