AbstractObjective:TostrengthentheriskmanagementoftheentireinsulindeploymentinPIVAS.Methods:Toconductthehospital

failuremodeandeffectanalysis(HFMEA)inthecompleteprocessofinsulindeploymentinPIVAS.Selectfailuremodeswithmedium

andaboverisklevels,sortoutandanalyzetheirfailurecauses,proposerectificationmeasures,andperformHFMEAagainoneachfailure

modeafterrectification.Results:InthewholeprocessofPIVASinsulindeployment,17failuremodesofhighandmediumrisklevels

wereselected,andrectificationsweremadefromseveralmajoraspects,suchasprocesses,HISsystem,andqualitycontrolmanagement.

Afterintervention,thetotalRPNvalueofthe17failuremodesdecreasedfrom1080to356,whichwasadecreaseof67.0%.

Conclusion:ApplyingHFMEAtotheriskmanagementofPIVASinsulinthroughouttheentireprocesscaneffectivelyimprovethe

safetyofinsulindeployment,andstrengthentheriskmanagementofPIVASinsulindeploymentthroughouttheentireprocess.

摘要目的：加强静脉用药调配中心（PIVAS）胰岛素调配全流程风险管理。方法：对PIVAS胰岛素调配全流程中各个环节进行医

疗失效模式与效应分析（HFMEA）。选出中等及以上风险等级的失效模式，整理分析其失效原因，提出落实整改措施，并对整改后

的各失效模式再次进行HFMEA。结果：在PIVAS胰岛素调配全流程中，共找出17项高等和中等风险的失效模式，并从流程、医院

信息管理系统（HIS）和质量控制管理等几个主要方面进行整改。干预后，17项失效模式合计风险优先指数（RPN）由1080分降至

356分，降幅67.0%。结论：将HFMEA应用于PIVAS胰岛素全流程
