重症脑卒中患者日常生活活动能力的调查研究 秦 芳1，王惠珍1，廖 黎2 （1，南方医科大学护理学院；2，南方医院神经内科，广东广州 510515） 摘 要 目的 探讨并分析重症脑卒中患者日常生活活动能力（ADL）及其影响因素。方法 采用日常生活能力评定量表（BI指数量表）对某医院神经内科33位重症脑卒中患者于入NICU，出NICU，出NICU后一个月，三个月，六个月分别进行调查。结果 重症脑卒中患者出NICU后六个月的BI指数得分最高（59.03±33.58）；入NICU时的APACHⅡ评分与患者出NICU后一个月，三个月，六个月的BI指数得分显著相关（P＜0.01）。患者的康复理疗对其出NICU后的BI指数得分具有影响（P＜0.05）。结论 随着时间的递进，重症脑卒中患者的BI指数得分越来越高，尤其是其出NICU后的一个月到三个月内增高幅度较大。应加强对重症脑卒中患者出NICU后日常生活的关注，促进肢体功能等方面的恢复，有意识提高患者的ADL，进而提高其生存质量。 关键词：重症脑卒中患者；日常生活活动能力；影响因素 The survey research of the activities of daily living of the patients with severe stroke Qinfang1，Wang huizhen1，Liaoli2 （1，The nursing school of SMedical university；2， of Nanfang hospital ，Guangdong Province，guangzhou 510515） Abstract Objective To explore and analyze the activities of daily living (ADL) and its impact factors of the patients with severe stroke. Methods Investigated 33 patients with stroke in the department of Neurology,one Hospital,with everyday functions rating scale (BI index scale).And every patient was investigated three times, respectively one month，three months and six months after they left the NICU. Results The BI index score was highest (59.03 ± 33.58)six months after the patients with severe stroke left the NICU; and the APACH Ⅱ score measured when the patients came into the NICU was closely related to the BI index score measured respectively one month，three months and six months after they left the NICU(P0.01).and the patients rehabilitation therapy has an significantly impact on the BI index score(P0.05). Conclusion As time Progressive, the patients with severe stroke BI index score is increasing high, and the big margin especially appears in a month to three months out of NICU.We should strengthen the attention of the the patients with severe stroke daily life,and promote the recovery of limb function, improve the patients ADL consciously and further improve their quality of life.
