第 43卷第 2期第 153页 华 中科技大学学报 (医学版) Vo1．43 NO．2 P．153 2014年 4月 ActaMedUnivSciTechnolHuazhong Apr． 2014 SALL4在生殖细胞肿瘤 中的表达及其临床应用价值 孟 丽 ， 杨子萱 ， 朱朋成 ， 敖启林 武汉市中心医院麻醉科 ，武汉 430014 。华中科技大学 同济医学院附属同济医院病理研究所 ，武汉 430030 摘要 ：目的 探讨SALL4在生殖细胞肿瘤中的表达变化及其临床应用价值。方法 收集精原细胞瘤、无性细胞瘤、 卵黄囊瘤、胚胎性癌及卵巢其他非生殖细胞肿瘤等病例共 88例。利用免疫组织化学染色法 (EnVision两步法)，对上述 各类生殖系统疾病进行 SALL4表达检测，结合其不同组织学类型，分析其表达情况。结果 ①SALL4在性腺生殖细胞 肿瘤和性腺外生殖细胞肿瘤 中均有不 同程度表达 。其 中，在 53例性腺生殖细胞肿瘤 中 5O例呈阳性，阳性表达率为 94．3 。在 9例性腺外生殖细胞肿瘤中均有表达，阳性表达率为 100 。②sALL4在不同的性腺生殖细胞肿瘤中表达 率不同。其中，2o例精原细胞瘤中阳性 19例，阳性表达率为 95 ；20例卵黄囊瘤均为阳性 ；8例无性细胞瘤中阳性 6 例，阳性率为75 ；5例胚胎性癌中均为阳性。③SALL4在生殖系统 26例非生殖细胞肿瘤中，如浆液性乳头状囊腺癌、 黏液性乳头状囊腺癌 、卵泡膜纤维瘤和颗粒细胞瘤等 ，未见阳性表达 。结论 SALL4是一种新型的生殖细胞肿瘤诊断 标记物，具有较好的敏感性和特异性 ，在生殖细胞肿瘤的病理诊断和鉴别诊断中具有重要价值。 关键词：sALL4； 生殖细胞肿瘤 ； 免疫组织化学 ； 鉴别诊断 中图分类号 ：R32l_1 DOI：10．3870／j．issn．1672-0741．2014．02．007 ExpressionofSALL4inGerm CellTumorsandItsApplication M engLi ，YangZixuan。，ZhuPengcheng△etal DepartmentofA Ps Ps0Z0g ，WuhanCentralHospital，Wuhan430014，China 。InstituteofPathology，TongJiHospital，TongJiMedicalCollege，HuazhongUniversity ofScienceandTechnology，Wuhan430030，China Abstract Objective ToinvestigatetheexpressionofSALL4ingerm cel1tumors(GCTs)anditsclinicalapplicationval— ue．Methods A totalof88samplesofGCTs(includingclassicseminoma，dysgerminoma，yolksactumor，embryonalcarcinoma) andothernon-germ celltumorswereimmunohistochemicallydetectedfortheexpressionofSALL4．Andtheresultswerearia— lyzedbasedonthehistologicaltype．Results ①SALL4stainingofdifferentintensitiescanbeobservedingonadaIandextrago— nadalGCTs．SALL4waspositivelyexpressedin50of53 (94．3 )gonadalGCTs；strongSALL4stainingwasseeninal19ex— tragonadalGCTs．②IngonadalGCTs，19of20(95 )classicseminomashadpositiveSALL4staining；strongSAL
