有机产品认证文件资料清单 Documentation List for Organic Product Certification （野生植物采集和加工） (Wild Plant Collection Processing) 申请单位应提供以下文件资料并按序号编排、装订后提交： The applicant shall submit the following documents bound in the order as below:  野生植物采集 Wild Plant Collection 《有机产品认证申请书》Application for Organic Product Certification 2.《有机产品认证调查表》（野生植物采集）Organic Product Certification Questionnaire (Wild Plant Collection) 3. 营业执照副本复印件和组织机构代码证复印件Copy of business license duplicate and copy of organization code certificate 4. 政府主管部门颁发的野生资源采集许可证（需包含采集面积、品种、数量）Wild resource collection permit that shows the area, species and quantity of collection issued by a government organ in charge 5. 政府主管部门出具的36个月内采集区域未使用过化学农药等违禁物质的证明Certificate that no banned substance, such as chemical pesticide, has been applied within the area of collection in recent 36 months issued by a government organ in charge 6. 采集合同、采集户清单、基地与农户管理制度Collection contract, list of collectors and base and farmer management system 要求：Requirements:采集户清单应至少包含姓名、身份证号、采集数量、采集区域等；The list of collectors shall show at least name, ID Card No., quantity collected and area of collection; (2)农户管理制度应体现有机生产的相关要求和对农户有效的管理。 management shall show organic production requirements and effective management of farmers. 7. 产地（采集）区域位置图Location plan of the place of origin (collection) 要求：Requirements:基地所在地的行政图（市、县或乡的行政图，表明基地所在的位置）； Administrative map of the location of the base (the administrative map of the city, county or town that shows the location of the base);采集区域分布图（多区域、分布不集中情况下，提供包含全部采集基地分布情况的地图）；Collection area distribution diagram (the map showing the distribution of all collection bases shall be provided in case of several bases or disperse distribution); (3)采集区域图（标明每个采集区域的形状、大小、采集产品名称及缓冲带情况）。Collection area map (showing the shape, size, name of product collected and buffer strip of every collection area) 8. 有机产品生产、加工规划Organic product production and processing plan 9. 质量管理手册Quality management manual 要求：Requirement: 请参考GB/T19630.4《有机产品》第4部分：管理体系4.2.4
