The T rad it ional M ed icine H osp ital of Jiangx i, 330006 Abstract Objective To investigate a simple, safe and effective met hod of treatment for open fracture of the extremities with bone de fect. Methods Ninety eight patients w ith open fracture combined w ith bone defect were treated w ith debridement, appropriate fixa tion and the follow ing treatment : 1. To implant (autosetting calcium phosphate cement, ACPC) impregnated with antibiotics; 2. To implant bone autograft mixed w ith antibiotics; 3. To close the w ound at early stage and implant autologous bone with vascular pedicle and ACPC impregnated with antibiotics at late stage. Results There were no significant difference in treatment of the open and mildly and intermediately contaminated facture with less bone defect betw een wit h the ACPC impregnated with antibiotics and with bone auto graft mixed with ant ibiotics. The open and quite severely contaminated fracture with bone defect w as indicated for treatment w ith the ACPC impregnated wit h antibiotics. The open and severely contaminated fracture with more than 5 cm segmental defect was indicated for t he treatment w ith debridement at early stage and w ith autologous bone grafting with vascular pedicle and ACPC impregnated with antibi otics at late stage. Conclusion The approximate treatment for the open fracture with bone defect should depend on the extent of the bone defect and the contamination in case multiple operations
