损伤肠管的异位节育器的因素及处理方法 1，2 1* 2 白昌民 ,于学文 ,何菊仙 作者单位：1.710061陕西 西安，西安交通大学第一附属医院预防保健科；2.710003陕西 西安，陕西省妇幼保健院科室妇三科 作者简介：白昌民，本科毕业于白求恩医科大学 副主任医师 妇科微创治疗 （附6例病例报告） 【摘 要】目的 分析损伤肠管的异位节育器（IUD）形成的因素，探讨损伤肠管的异位IUD的处理方法。 方法 回顾分析陕西省妇幼保健院6例异位IUD损伤肠管最后经腹腔镜或开腹取出的病例。结果 6例患者1 例为闭合式圆型IUD，5例为非闭合式V型爱母IUD，3例宫腔镜检查未见IUD，3例宫腔内可见2-10mm部分IUD。 6例患者均进行了腹腔镜取IUD术，4例腹腔镜成功取出，2例转开腹取出，6例患者均愈合良好。结论 腹腔 镜处理损伤肠管的异位IUD可做为首选，宫腔镜仅见少部分IUD时，不要盲目取出，先行腹腔镜探查并取出， 必要时中转开腹取出。 【关键词】IUD异位；肠管损伤；非闭合IUD；腹腔镜；宫腔镜 Factors of Loops damage by ectopic IUD and management Attached A report about 6 cases 1,2 1* 2 BAIChang-min ,YUXue-wen ,HEJu-xian （1.Department of pveventive and health in the first Affiliated Hospital of xian JiaotongUniversity Xi Shaanxi , , ’an 710061, P.R.China 2.DepartmentofGynecology Ⅲ,ShaanxiMaternalandChildHealthHospital,Xi’an Shaanxi710003 P.R.China, ∗ Correspondingauthor,E-mail:yuxuewen2007@163.com 【Abstract】 Objective:To investigatethefactorsof loopsdamageby ectopic IUD,and to find out suitable ways to cope with it. Methods: Six cases of ectopic IUD were studied retrospectively from maternity and child-care hospital in shanxi province, which revealed loops damage by IUD dystopy and finally the IUD were taken out by laparoscopy or open abdomen surgery. Results：Among6cases,onebelongedtoclosed-modelIUD,theother5cases werenon closed-model IUDwithV-shape(MCUIUD).Hysteroscopy examination indicatednothing in3 caseswhile parts of IUDappeared varying from 2cm 1 to 10cms in another 3 cases. All the 6 patients recovered well after IUD were removed by laparoscopy in 4 cases or by open abdomen surgery in 2 cases.
