·58· 《中国病案》2015 年第 16 卷第 4 期 某医院 5 年住院患者疾病构成及相关费用分析 400020 重庆市 解放军324 医院信息科 李毅志 邹 频 施 萍 摘要 目的 通过对住院患者的疾病构成及相关费用分析，为科学决策提供依据。方法 依托医院信息 系统，按照ICD-10国际疾病分类法，以第一诊断作为依据，对2009年-2013年医院住院患者进行数据分析 挖掘。结果 5年住院患者前三位分别是支气管肺炎、冠状动脉粥样硬化性心脏病、高血压Ⅲ级，合计比重 分别为3.28%、2.11%、1.81%；5年人均费用都排在前三位的分别是精神分裂症、冠状动脉粥样硬化性心 脏病、高血压Ⅲ级，增幅为108.5%、89.9%、70%，药品费比例较高。结论 针对这些常见病，要加强特 色科室建设，制定疾病防治策略，多开展新业务新技术，降低药品费比例，控制医疗费增长，提高医院的 核心竞争力。 关键词 住院患者；疾病构成；费用构成；统计分析 Analysis on Disease Constitution and Fee Structure of a Hospital in the Past 5 Years Li Yizhi, Zou Pin, Shi Ping, Information Department, 324 Hospital of PLA, Chongqing 400020, China Abstract Objective To analyze disease constitution and fee structure of inpatients, provide a basis for scientific decision-making. Method Relying on the hospital information system, analyze the first diagnosis based on the ICD-10 International Classification of Diseases for analyzing the inpatient data of the year form 2009 to 2013. Results The top three diseases are bronchial pneumonia, coronary atherosclerotic heart disease, and hypertension level Ⅲ. The proportion of these disease are 3.28%,2.11%,1.81%; the top three of the per capita cost in these five years are schizophrenia, coronary atherosclerotic heart disease, hypertension level Ⅲ, the increasing rate of these disease are 108.5%, 89.9%,70%, and drug costs occupied a higher proportion. Conclusion For these common diseases, corresponding departments should develop disease prevention strategies, new technologies, lower drug cost proportion and more new business to control health care costs and improve the core competitiveness of the hospital.
