For clinical old people femoral neck fracture fixation for selection and provide a basis for future research. Methods: Collected from July 2010 to August 2011 in shandong university of traditional Chinese medicine hospital orthopaedic hospital ward and accept the surgical treatment of patients ，were selected the clinical research of close reduction and percutaneous internal fixation by compressive screw in treatment of femoral neck fractures of Garden Ⅲ，Ⅳ in elderly patients were 63 cases of cases. 27 cases of male,female cases 36. The age is more than 56 to 79 years old, average 65.35 years old, 56 to 65 years old have 32 cases, 66 to 75 years old have 19 cases, 76 to79 years old have 12 cases. The left femur 34 cases of cervical fracture, 29 cases of right. In typing Garden: 37 cases of type Ⅲ, 26 cases of type Ⅳ. Postoperative follow-up of 8-16 months. Results: 63 cases of fracture in this group of patients, have one case of fracture nonunion,the incidence rate was 1.6%, and have two cases of avascular necrosis of femoral head occurs, the incidence rate was 9.5%, and the method of the overall cure rate was 88.9%.
