胸骨上入路完全腔镜下甲状腺手术的应用研究 叶劲松，侯东生，陈光春，张程鹏 本文档由【中文word文档库】提供，转载分发敬请保留本信息； 中文word文档库免费提供海量范文、教育、学习、政策、报告和经济类word文档。 （广州医学院第一附属医院微创外科中心，广东 广州 510230） 【摘要】 目的 探讨胸骨上入路完全腔镜下甲状腺手术的安全性及技术要点。 方法 于2002年10月到2009年12月对57例甲状腺良性病变施行胸骨上入路完全腔镜下甲状腺手术。其中结节性甲状腺肿30例，甲状腺腺瘤15例，甲状腺囊肿12例。 结果 所有手术均获成功，手术时间为90~180 min，无中转开放手术。术后无大出血、声音嘶哑、手足抽搐等严重并发症发生。随访6～18月无复发，颈部仅留微小疤痕，美容效果满意。 结论 胸骨上入路完全腔镜下甲状腺手术是一种安全、微创、美容的手术方法。手术有效操作空间的建立、超声刀的正确使用以及熟练的腔镜操作技巧是手术成功的重要保障。 【关键词】 胸骨上入路；腔镜；甲状腺手术 The clinical application of endoscopic thyroidectomy via the superior of sternum.Ye Jin-song，Hou Dong-sheng， Chen Guang-chun，et al. Center of Mini-invasive Surgery， the First Affiliated Hospital of Guangzhou Medical College, Guangzhou, China, 510230 【Abstract】 Objective To explore the safety and technique difficulties of endoscopic thyroidectomy via the superior of sternum .Methods From October 2002 to December 2009，57 patients with benign thyroid diseases underwent endoscopic thyroidectomy via the superior of sternum, including 30 cases of nodular goiter ，15 cases of thyroid adenoma ，12cases of thyroid cyst. Results 57 cases were completed successfully without complication such as bleeding or damage of nerve and parathyroid gland .The operative time was ranged from 90 to 180 min .No recurrence was found in 57 cases after 6-18 months following up .All the patients were satisfied with the cosmetic result. Conclusion Endoscopic thyroidectomy via the superior of sternum is a safe，mini-invasive，convenient way .Keeping the adequate handling space，applying the ultrasound knife correctly and having good endoscopic technique are essential to perform endoscopic thyroidectomy successfully. 【Key Words】 The superior of sternum approach；Endoscopic；Thyroidectomy 传统甲状腺手术不可避免地在颈部留下明显的手术疤痕，影响美容效果。腔镜甲状腺手术可以使手术切口微小化甚至隐蔽起来，从而获得良好的美容效果，越来越受到中青年患者，尤其是青年女性患者的青睐。我科于2002年10月到2009年12月成功为57例甲状腺患者施行胸骨上入路完全腔镜下甲状腺手术，并取得良好的手术及美容效果，报道如下资料与方法 11 临床资料本组57例中，男21例，女性36例，年龄18~65岁。术前均经B超或CT诊断为甲状腺良性病变，并排除颈部淋巴结肿大，所有病例甲状腺功能正常。其中，结节性甲状腺肿30例，甲状腺腺瘤15
