摘 要 目的：探讨在HBV相关性肝脏疾病中外周血的骨桥蛋白(OPN)、白介素-12及白介素-18的表达水平及其临床意义。 方法：应用ELISA方法测定15例健康献血者（对照组）和62例HBV相关性肝脏疾病患者和的外周血中的白介素-12、白介素-18及骨桥蛋白的表达水平，并分析其相关性。 结果：骨桥蛋白在肝炎组、肝硬化组以及原发性肝癌组中的表达水平均显著高于正常对照组的表达水平，与正常对照组相比 P 0.05；骨桥蛋白在不同HBV相关性疾病中原发性肝癌组表达水平最高，之后为肝硬化组，肝炎组的表达水平最少；慢性乙型肝炎患者血清按不同胆红素水平分为三组，血清中TBIL水平与OPN、IL-12及18表达呈正相关（r＝0.45，P＜0.01；r＝0.48，P＜0.05；r＝0.79，P＜0.05）。在HBV-DNA复制程度对外周血骨桥蛋白及白介素-18表达水平无明显差异，但与白介素-12表达水平有显著差异。 结论：骨桥蛋白、白介素-12及白介素-18在HBV相关性肝脏疾病中的表达不同，与其发生、发展及预后有关。 关键词：骨桥蛋白；白介素-12；白介素-18；Th1/Th2；ELISA； Abstract Objective: To investigate HBV-related liver disease in the peripheral blood of expression levels of osteopontin (OPN), interleukin-12 and interleukin-18 and its clinical significance. Methods: In 62 patients with HBV -related liver disease patients and 15 healthy blood donors ( control group ) in the peripheral blood of expression levels of osteopontin by ELISA.interleukin-12 and interleukin-18 with ELISA were measured to analyze the expression level,relationship with HBV -related liver disease ,and analyze its relevance. Results: The expression level of osteopontin in primary liver cancer , cirrhosis and hepatitis group expression levels were significantly higher than of the control group compared with the control group P 0.05; level of expression of osteopontin in different HBV -related primary liver cancer diseases was higher than it in liver cirrhosis, and level of expression of osteopontin in liver cirrhosis was higher than it in hepatitis ;Patients with chronic hepatitis B serum bilirubin levels were divided into three different groups , serum TBIL levels and OPN, IL-12 and 18 expression was positively correlated (r = 0.45, P 0.01; r = 0.48, P 0.05; r = 0.79, P 0.05). In HBV-DNA replication and no significant difference in peripheral blood and osteopontin expression levels of interleukin-18 , but there are significant differences in the expression levels of interleukin-12 . Conclusion: Expression of Osteopontin ,interleukin-12 an
