Application Form for Qualifications Assessment 學歷評估申請表格 Applicant may submit the completed application form with assessment fee by mail, in person or by an authorised representative to our office. If applicant c..hooses to submit the form in person or by an authorised representative, please make an appointment before the submission through our Online Appointment Booking System (.hk). An Applicant without an appointment is required to drop-off the application at our reception counter. Late Appointee will only be served within the original reserved timeslot.
