PAGE \* MERGEFORMAT 18 Percutaneous endoscopic transforaminal approach extreme lateral lumbar disc herniation treatment Of: Zhang Changjiang, Ren Wenjie, Wang Mingjun, Zhang Kai, Li Hongke, Dongsheng Li, Shuai [Abstract] Objective To investigate the percutaneous endoscopic transforaminal approach for treatment of extreme lateral lumbar disc herniation (FLDH) the safety and efficacy. Methods From December 2007 to March 2009, 16 patients were diagnosed as FLDH using local anesthesia, in the C-arm X-ray machine under the guidance of percutaneous endoscopic transforaminal approach technology, gasification resection under direct vision of the far lateral nerve root compression prominent nucleus pulposus, nerve decompression and release involved roots. using visual analog scales (VAS) preoperative and postoperative 1 month and final follow up in patients with pain conditions score, the final follow up while using the modified Macnab criteria for efficacy evaluation. Results All the operations were successful, the operation time of 40 ~ 90 min, an average of 65 min; blood loss was 20 ~ 180 ml, an average of 40 ml; postoperative complications were not any. followed up for 2 to 12 months, an average of 4 months, assessed by VAS score, after 1 months and final follow up compared with the preoperative VAS score differences were statistically significant (P lt;0.01); assessed by Macnab criteria, the group were excellent in 12 cases, good in 2 cases and 2 cases. Conclusion The percutaneous endoscopic transforaminal approach FLDH safe and effective minimally invasive treatment of surgery, are worth promoting.
