Objective. To retrospectively evaluate ANCA testing in a cohort of unselected Greek in- and outpatients. Methods. In 10803 consecutive serum samples, ANCA were tested by indirect immuno?uorescence (IIF) and ELISA. ELISA in inpatients was performedonlyonIIFpositivesera.Results.Lowprevalence(6.0%)ofIIFpositivesampleswasobserved.Amongthesesamples, 63.5%presentedperinuclear(p-ANCA),9.3%cytoplasmic(c-ANCA)and27.2%atypical(x-ANCA)pattern.16.1%ofp-ANCA wereantimyeloperoxidase(anti-MPO)positive,whereas68.3%ofc-ANCAwereantiproteinase-3(anti-PR3)positive.Only17 IIF negative outpatients’ samples were ELISA positive. ANCA-associated vasculitides (AAV), connective tissue disorders and gastrointestinaldisordersrepresented20.5%,23.9%,and21.2%ofpositiveresults,respectively.AAVpatientsexhibitedhigherrates ofMPO/PR3speci?citycomparedtonon-AAV(93.8%versus8%).Conclusions.This?rstpaperonGreekpatientssupportsthat screeningforANCAbyIIFandcon?rmingpositiveresultsbyELISAminimizelaboratorychargeswithoutsacri?cingdiagnostic accuracy. 1.Introduction areoftenobservedinWGpatients,whilep-ANCApredom- inantly bind to MPO and are common in patients with MPA and CSS [1].
